Purpose of the post: Dublin South West Integrated Care Programme for Older Persons Department Description The Older Persons Service Model sets out the end-to-end service model for the identification and management of frail older adults with complex care needs. The focus is on providing an end-to-end pathway that will reduce admissions to acute hospitals by providing access to diagnostics and specialist services in the ambulatory care hubs in a timely manner. For patients who require hospital admission, the emphasis is on minimising the hospital length of stay, with the provision of post-discharge follow up and support for people in the community and in their own homes, where required. A shared local governance structure across the local acute hospitals and the associated integrated healthcare area will ensure the development of a fully integrated service and end-to-end pathway. The Dublin South West (DSW) Integrated Care Programme for Older Persons (ICPOP) Community Specialist Team (CST) is a specialist transdisciplinary service primarily targeting and managing the complex care needs of the older person with multiple co-morbidities across a continuum of care. The overall aims of the service are to: • Provide a specialist geriatric opinion using a multidisciplinary approach to support older people with complex care needs. • Develop a person-centred care planning approach that supports robust and timely communication across care settings. • Support appropriate and timely reduction of Emergency Department (ED) attendance through the development of care pathways that support GPs and others in assessment of older people with escalating care needs. • Provide support and education to the older person, carers and healthcare professionals. Clinicians in the DSW ICPOP CST, will work closely with the Consultant Geriatrician(s) and ICPOP operational team lead to develop and implement appropriate care pathways while working with primary care, community care and acute care services. DSW ICPOP CSTs utilise a case management approach to assess and meet the complex care needs of frail older persons: • All clinicians on the DSW ICPOP CSTs are responsible for case management. • This includes the completion of a comprehensive geriatric assessment with each patient in their own home. • The case manager works closely with H.S.E. primary care & community services such as PHN, HSCP colleagues, respite, home support services, nursing home support office, day care services and psychiatry of later life. • The case manager has extensive links with our community and voluntary partners such as South Dublin County Council, SAGE Advocacy, ALONE, Carers Ireland, Alzheimer’s Society of Ireland, Gardaí. The DSW ICPOP CSTs liaise and work closely with our acute hospital colleagues including the frailty at the front door teams, geriatric day unit (Charlie O’Toole Unit), inpatient gerontology staff. The DSW ICPOP CSTs are based out of our new Integrated Care Hub in Clondalkin Primary Care Centre, where we are co-located with the Integrated Care Programme for Chronic Disease CST, where we have clinical space and gyms. Consultant Haematologist With A Special Interest In Paediatric Haematopoietic Stem Cell Transplantation
Scope of the Role: The National Children’s Cancer Service (NCCS) at Children’s Health Ireland (CHI) , Crumlin provides diagnosis and treatment to all children and adolescents (0–15 yrs, 364 days) with cancer in Ireland. This comprises those with solid tumors (Oncology) and those with Haematological malignancies (Leukaemia and lymphoma). The NCCS also incorporates the National Centre for Paediatric Haematopoietic Stem Cell Transplant (HSCT) and is aligned with the National Cancer Control Programme. The activity of the NCCS has significantly increased in both size and complexity over the past number of years. Comprehensive care for children and adolescents with cancer is delivered by robust clinically governed programmes and patient pathways. Treatment for Paediatric Leukaemia and lymphoma (collectively referred to as Haematological Malignancies) is planned according to internationally recognised best practice, and is delivered according to international treatment protocols. The majority of patients with Haematological Malignancies are enrolled on Clinical Trials and treatment is delivered by robust clinically governed programmes. This requires on-going collaborative discussions at MDT level with other CCLG centres. Consultant Paediatrician Special Interested Peri-operative Care
Purpose of role: The Part time Consultant Paediatrician role at NOHC is to provide acute care for children with general and complex presentations needing surgical intervention. They will also assist in the development of a peri-operative service in NOHC. In the interim they will partake on the general paediatric on call rota in NOHC. They will also partake in acute and out-patient General Paediatrics work in CHI. The post holder(s) will liaise between NOHC and CHI to ensure appropriate governance, patient selection, staff training, review and development of protocols, quality assurance and an on-call commitment to paediatric inpatients. Expected sessions on site in Cappagh would include attendance at the 3 weekly pre and post operative conference, attendance at quarterly QI conference and attendance at appropriate committee meetings. The post holders would be expected to do 1 to 2 ward rounds per week. The on-call commitment will cover a ward of 13 beds in total, with a normal maximum of about 5-6 inpatients overnight currently. These are all preselected, low medical complexity orthopaedic cases who have been pre-assessed. Most are single overnight stays for analgesia with 4-5 days at the upper range for limb deformity cases and scoliosis. The ward is staffed by experienced, fully trained paediatric nurses and NCHD cover is currently provided by an in-house orthopaedic registrar from CHI. Locum Consultant Paediatric Nephrologist
Purpose of role: The newly appointed consultant will integrate with established teams at both TSCUH and OLCHC. This is a locum post, being filled to maintain current service levels. The appointee will deliver general nephrology outpatient clinics at both sites, in addition to integrating into our established specialist nephrology clinics which are run on a collaborative basis (e.g. transplant and dialysis clinics; nephro-urology; specialist nephrology follow up clinics). The appointee will play a full role in delivering inpatient ward rounds; and providing a consultation service at both sites. International Clinical Fellow In Paediatric Critical Care Medicine
International Paediatric Critical Care Fellowship The Division of Paediatric Critical Care Medicine of Children’s Health Ireland (CHI) has devised a 12 to 24 month fellowship programme to meet the requirements of senior trainees from around the globe seeking to qualify as specialists in Paediatric Critical Care Medicine. The Division of Paediatric Critical Care Medicine CHI at Crumlin houses a 23 bedded Paediatric Intensive Care Unit (PICU) incorporating both a General and Cardiac ICU. It is one of the largest ICUs in the UK and Ireland with approximately 1,000 admissions per annum and supports multiple national sub-specialist services including cardiology, cardiothoracic surgery (at quaternary level for the Island of Ireland), oncology, gastroenterology, respiratory medicine, endocrinology, neurology and ENT. Furthermore, it houses Ireland’s only extracorporeal life support (ECLS) service for children. The PICU at CHI Temple Street is a 9 bedded general unit which supports the national metabolic and neurosurgical services amongst others and facilitates the national renal transplant programme. It receives over 400 admissions yearly. Both units staff the Irish Paediatric Acute Transport Service (IPATS), which stabilises and retrieves patients referred from throughout Ireland, by both land and air. Eligibility Criteria Applicants must have completed at least 2 years of subspecialty training in the field of paediatrics, anaesthesiology or emergency medicine and 6 months of tertiary level neonatal, paediatric or adult intensive care.
