Purpose of the post: Dublin South West Integrated Care Programme for Older Persons Department Description The Older Persons Service Model sets out the end-to-end service model for the identification and management of frail older adults with complex care needs. The focus is on providing an end-to-end pathway that will reduce admissions to acute hospitals by providing access to diagnostics and specialist services in the ambulatory care hubs in a timely manner. For patients who require hospital admission, the emphasis is on minimising the hospital length of stay, with the provision of post-discharge follow up and support for people in the community and in their own homes, where required. A shared local governance structure across the local acute hospitals and the associated integrated healthcare area will ensure the development of a fully integrated service and end-to-end pathway. The Dublin South West (DSW) Integrated Care Programme for Older Persons (ICPOP) Community Specialist Team (CST) is a specialist transdisciplinary service primarily targeting and managing the complex care needs of the older person with multiple co-morbidities across a continuum of care. The overall aims of the service are to: • Provide a specialist geriatric opinion using a multidisciplinary approach to support older people with complex care needs. • Develop a person-centred care planning approach that supports robust and timely communication across care settings. • Support appropriate and timely reduction of Emergency Department (ED) attendance through the development of care pathways that support GPs and others in assessment of older people with escalating care needs. • Provide support and education to the older person, carers and healthcare professionals. Clinicians in the DSW ICPOP CST, will work closely with the Consultant Geriatrician(s) and ICPOP operational team lead to develop and implement appropriate care pathways while working with primary care, community care and acute care services. DSW ICPOP CSTs utilise a case management approach to assess and meet the complex care needs of frail older persons: • All clinicians on the DSW ICPOP CSTs are responsible for case management. • This includes the completion of a comprehensive geriatric assessment with each patient in their own home. • The case manager works closely with H.S.E. primary care & community services such as PHN, HSCP colleagues, respite, home support services, nursing home support office, day care services and psychiatry of later life. • The case manager has extensive links with our community and voluntary partners such as South Dublin County Council, SAGE Advocacy, ALONE, Carers Ireland, Alzheimer’s Society of Ireland, Gardaí. The DSW ICPOP CSTs liaise and work closely with our acute hospital colleagues including the frailty at the front door teams, geriatric day unit (Charlie O’Toole Unit), inpatient gerontology staff. The DSW ICPOP CSTs are based out of our new Integrated Care Hub in Clondalkin Primary Care Centre, where we are co-located with the Integrated Care Programme for Chronic Disease CST, where we have clinical space and gyms. We have office space in Russell Centre, Tallaght Cross. Advanced Nurse Practitioner
(i)Registered Advanced Nurse Practitioner (RANP) Care of the Older Adult (ID) Or (ii)Candidate Advanced Nurse Practitioner (cANP) Care of the Older Adult (ID) Horizons is seeking to recruit a highly motivated and enthusiastic Advanced Nurse Practitioner (Care of the Older Adult) . The overall purpose of the post is to provide safe, timely, evidenced based nurse-led care to service-users at an advanced nursing level. This involves undertaking and documenting a complete episode of service-users care (assess, diagnose, plan, treat and discharge service-users) according to collaboratively agreed protocols and scope of practice in the clinical setting; demonstrating advanced clinical and theoretical knowledge, critical thinking, clinical leadership and decision-making skills. 1. Statutory Registration, Professional Qualifications, Experience, etc The following criteria apply for registration as an ANP/AMP with NMBI. You must: (i) Be a registered nurse/midwife on the active Register of Nurses and Midwives held by Nursing and Midwifery Board of Ireland (NMBI) or be eligible to be so registered. AND (ii) Be registered in the Prescribers Division. The cANP (Pathway 2) applicant will need to complete the Nurse Prescribing of Medicinal Products Certificate. AND (iii) Have a minimum of 5 years’ post registration full time experience or an aggregate of 5 years’ full-time experience in the division of Intellectual Disability (Other Nursing disciplines with a minimum of 5 years’ post registration experience working in the area of intellectual disability may apply) AND (iv) Have at least three years of recent post-registration clinical experience, with one year of equivalent full-time experience in the specific area of practice. OR If the applicant does not possess the relevant specialist experience, they will be supported to attain one year’s specialist experience. AND (v) Hold a Master’s degree (or higher) in nursing/midwifery or a Master’s degree which is relevant, or applicable, to the advanced field of practice. The Master’s programme must be at Level 9 on the National Framework of Qualifications (Quality & Qualifications Ireland), or equivalent. Educational preparation must include at least three modular components pertaining to the relevant area of advanced practice, in addition to clinical practicum. In recognition of services that span several patient/client groups and/or division(s) of the Register, provide evidence of validated competencies relevant to the context of practice. Alternatively provide written evidence from the Higher Education Institute that they have achieved the number of ECTS credits equivalent to a Level 9 or higher standard, relevant to the specialist area of care (equivalent to 60 ECTS or above), and in line with the requirements for specialist practice as set out by the National Council for Nursing and Midwifery 4th ed (2008). OR If the applicant does not possess the relevant QQI NFQ Level 9 Qualification the applicant will be supported to undertake the required postgraduate education at QQI NFQ Level 9 qualification (equivalent to 60 ECTS or above) relevant to Care of the Older Adult (ID) and progress on Pathway Two. AND (vi) Be required to demonstrate that they have continuing professional development (CPD) relevant to Care of the Older Adult (ID) or will be supported to obtain the required CPD. AND (vii) Have the ability to practice safely and effectively fulfilling his/her professional responsibility within his/her scope of practice All of the above must be achieved within 2 years for the candidate to progress on Pathway Two to be eligible to register as a ANP. AND (b) Candidates must possess the requisite knowledge and ability, including a high standard of suitability and clinical, leadership, managerial and administrative capacity for the proper discharge of the duties of the office. 2. The successful applicant will work within an interdisciplinary team as a member of Children’s Disability Network Team 8, Central Cork. The post holders will support to children with complex needs, and their families. This will include children with physical disabilities, intellectual disabilities and autism. Responsibilities will include collaborative goal setting, individual and group intervention, assessment and monitoring of physical development and postural management equipment prescription to children with complex needs arising from a disability. The post holder will work across a variety of settings including the clinic, hydrotherapy pool, preschools, schools, special schools, children's homes and community settings. CDNT 8 work closely with CHO4 Physical Disability Support Services to assess and monitor the complex physical and orthopaedic needs of the children on the caseload (posture, muscle length and muscle tone, hip and spinal surveillance.)
