Overview of the Role The key features of the role are: The Finance Unit effectively reconciles and reports on all income, expenditure, maintaining appropriate accounting records of all financial transactions. Details of Service The Women & Infant’s Directorate for Maternity, Neonatology and Gynaecology is located in University Hospital Kerry, which is a Model 3 hospital. UHK has approximately 1,100 births per annum and has a tertiary referral centre to Cork University Maternity Hospital within Ireland South. Maternity Services at University Hospital Kerry are delivered within HSE South West, Ireland South Women’s and Infants Directorate which is based on the principles of partnership and integration and strongly supports the involvement of the multidisciplinary team in service provision. UHK is a site for clinical placements for student midwives and student nurses in undergraduate and postgraduate programmes delivered in partnership with University College Cork, University Of Limerick and Munster Technological University. Maternity Services, UHK incorporates the use of the MN-CMS (Maternal Newborn Clinical Management System) into care delivered at UHK. There are four Maternity Units in Ireland South Women and Infants Directorate – University Hospital Kerry, University Hospital Waterford, Cork University Maternity Hospital and Tipperary University Hospital - spanning four of the largest counties in the country. Maternity Services includes Neonatology and Gynaecology as well as Midwifery and Obstetrics. The Group’s Primary Academic partner is University College Cork. A wide range of educational programmes and research is conducted within all hospitals in the Group. The cAMP provides expert, autonomous care to women experiencing complications in pregnancy, with a particular focus on the diagnosis, management, and emotional support of women with threatened miscarriage, miscarriage, recurrent miscarriage (RM), and including ectopic pregnancy and pregnancies of unknown location (PUL), Mid trimester Pregnancy Loss and Stillbirth. The role includes clinical assessment, prescribing, and coordination of care within a multidisciplinary team. Purpose of the post The individual will undertake the academic preparation and develop the clinical and leadership skills, competencies and knowledge required to meet the criteria to be registered as a RAMP with NMBI. The scope of the cAMP role must reflect the incremental development of expertise and as such, the cAMP cannot deliver care as an autonomous practitioner. The individual will complete and submit the necessary documentation for registration as an RAMP with NMBI. The value of the nursing/midwifery contribution as a distinct profession must be safeguarded and articulated in the development of new services led by advanced nurse/midwife practitioners, complementing rather than replacing current services delivered by doctors (NMBI 2017 p.9). The overall purpose of the post is to provide safe, timely, evidenced based midwife-led care to patients at an advanced midwifery level. This involves undertaking and documenting a complete episode of patient care (assess, diagnose, plan, treat and discharge patients) according to collaboratively agreed protocols and scope of practice in the clinical setting; demonstrating advanced clinical and theoretical knowledge, critical thinking, clinical leadership and decision making skills. The advanced practice role demonstrates a high degree of knowledge, skill and experience that is applied within the nurse/midwife-patient/client relationship to achieve optimal outcomes through critical analysis, problem solving and accurate decision-making (NMBI, 2017). The cAMP will manage a caseload depending on service demand and acuity to include o Women with threatened miscarriage and miscarriage o Suspected or confirmed ectopic pregnancies. o Pregnancy of unknown location (PUL) o Post-management follow-up (following expectant, medical, or surgical management). o Women with recurrent miscarriage (RM) o Bereavement support, clinical triage, and referrals to gynaecology, mental health, and community services. o Clear, compassionate communication regarding results, next steps, and recovery expectations, when to seek urgent care, support and access in subsequent pregnancies. o Telephone or in-person follow-up consultations to assess physical and emotional recovery. o Referral of patients to gynaecology, mental health, bereavement counselling, fertility services or the pregnancy loss clinic as appropriate. o Escalation of care for women with ongoing symptoms e.g. retained pregnancy tissue or ongoing bleeding, molar pregnancy o The development of a pregnancy after loss clinic which will incorporate a pre conceptual clinic o The development of a home visit pathway for families following stillbirth Central to this is the provision of quality care, a safe environment and processes for women by the use of evidence based clinical guidelines that address patient expectations, promote wellness and evaluate care given. Whenever the Minister is of the opinion, however, that it would not be in the interests of the common good to seek such consent and he/she certifies accordingly, the Officer shall furnish all required information to a Registered Medical Practitioner authorised by the Minister. Where, however, the Officer is of the opinion that the nature of the case is such that he/she should inform the patient of the requirement of the Minister, he/she then shall be at liberty to do so. It is not however intended that this requirement will operate in a manner, which would detract from the patient’s character or reputation. · To issue, or arrange for the issue of, to or in respect of any patient under his /her care in the hospital or referred to him /her at a clinic and without payment by or on behalf of the HSE, any certificate which might reasonably be required by such patient regard to the state of his/ her health in connection with his /her normal employment, his/ her entitlement to benefit under the scheme of social insurance or assistance. · To supervise and be responsible for the clinical work and record keeping of Non-Consultant Hospital Doctors and as appropriate other staff and to instruct them in the proper performance of their duties. · To ensure that adequate supplies of such drugs, medicines, medical and surgical appliances as he/she considers necessary, will be available in his/her department of the hospital and to report to the HSE, arrangements for the custody of dangerous drugs etc., which he/she considers to be unsatisfactory. · To be responsible, under the Executive Clinical Director/Clinical Director, for the general management and running of hospital and community based departments or of such other psychiatric services operated by the HSE as may be assigned to him/her and to attend and be responsible for the care and treatment of patients concerned in such assignment. · To take a special interest in and be responsible, under the person in charge, for the general organisation of such aspects of the Mental Health Services of the HSE as may be assigned to him/her. · To be responsible for the maintenance of appropriate liaison arrangements between his/her department in the hospital or community services and the patients normal medical attendant. · To participate in a system of professional practice review to support the maintenance of service quality and standards. To deputise and supply cross-cover as appropriate for his/her colleagues in the event of service exigencies in agreement with the Executive Clinical Director/Clinical Director and management of the service. · To reside convenient to Cork Mental Health Services or at such place as may be approved by the Head of Mental Health Services, HSE South West. · To agree with his/her Executive Clinical Director/Clinical Director, the consultant colleagues to assume clinical responsibility for his/her patients during his/her absence and document same. He/she shall agree with the Executive Clinical Director/Clinical Director, prior to taking leave, which members of medical staff will be on-call and available during that time. · To undertake referral meetings, case reviews as required. · To write letters and communications to General Practitioners and other professionals as required. · To participate in various medical staff meetings within the HSE as necessary. · To provide lectures/training as appropriate and as required in the Hospital and Clinical Service areas for medical, nursing and other staff or students. · To perform duties in occasional emergencies and unforeseen circumstances on consultation with senior and junior colleagues. · To assist in the development of best practice protocols in line with emerging research and best practice. · To participate in activities such as Clinical Practice; Risk Management; Health and Safety; Financial; Administration and HR. · To provide advice, support and consultancy in a variety of settings to minimise risk. · To participate in regular Audit and Quality Assurance programmes to improve and maintain the high standards of service. · To participate in clinical audit, clinical governance and strategic planning. · To contribute to or conduct research in clinical or service areas, in accordance with approved policies, pertinent to the service and their own relevant experience. · To comply with reporting requirements and system developments e.g. Mental Health Commission etc and to provide data/activity as required for performance monitoring processes in the HSE South West. · To ensure compliance with all HSE National and Local policies and procedures. · To notify the appropriate officer of any case of infectious or suspected infectious disease in his/her Department and to furnish such particulars as may be required in each case and to carry out the preventative measure that may be necessary. · The person appointed will hold office under the terms and conditions of the revised consultant’s contract, which will determine that he/she will participate actively in the management of the unit, take cognisance of resource constraints and retain clinical autonomy while practicing. · To work with a multidisciplinary team providing assessment and therapeutic services and to have special administrative responsibility if required for the Service. · The person appointed will take an active role in the selection and recruitment of staff where required. · To be responsible for the maintenance of appropriate liaison arrangements between his/her unit and the patients normal medical attendants. · The person appointed to the office is a Designated Officer under the “Protections for Persons Reporting Child Abuse Act, 1998” and consistent with Children’s First Guidelines 2011 to receive reports of child abuse. Such officers will, on receiving a report of child abuse, formally notify the relevant Child and Family Services Manager (Tusla). · To prepare clinical and court reports pursuant to Section 25 of the Mental Health Act 2001. · To ensure that current Health and Safety Legislation is complied with in consultation with the Executive Clinical Director/Clinical Director, Area Director of Nursing and other responsible persons. · To participate and ensure that junior medical staff participate in regular Health &Safety exercise e.g. Fire Drill. · To perform such other duties appropriate to his/her office as may be assigned to him/her by the Head of Mental Health Services, HSE South West or other designated officers.
