Details of Service St Columcille’s Hospital (SCH), Loughlinstown is a Model 2 hospital with 114 inpatient beds, 3 medical observation beds, 8 day beds, a 9 bedded Medical Assessment Unit (MAU) and a 6 bedded Local Injuries Unit (IU). Located on the border of Wicklow, it has a national speciality in obesity management and is the location of the National Gender Service. The hospital also provides Diagnostic Radiological and Pathology services as well as Nutrition and Dietetics, Physiotherapy, Occupational Therapy, Speech & Language Therapy, Cardiac Diagnostic and Rehabilitation Services, Pulmonary Function Laboratory and Respiratory Services. The specific remit of this post is to staff and manage the Endocrine Clinics at the National Gender Service, with the support of the existing Consultant staff and multidisciplinary team. In addition, there is a requirement to provide medical on-call cover at St Columcille’s Hospital (SCH) on a 1 in 8 basis. When these core commitments have been met, there is opportunity to work in the Centre for Obesity Management at SCH, and to work in the busy Diabetes and General Endocrinology clinics. Reporting Relationship The Consultant reporting relationship and accountability for the discharge of his/her contract is: General Manager through his/her Clinical Director Purpose of the Post To staff and manage the Endocrine Clinics at the National Gender Service, based on the Campus of St. Columcille’s Hospital. The post holder will work 39 hour per week and part take in a 1:8 on call at St. Columcilles Hospital Principal Duties and Responsibilities Clinical/Professional Responsibilities · To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the Employer. · To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services. · To work within the framework of the hospital level of service as determined by the Employer, Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply. · To co-operate with the expeditious implementation of the Disciplinary Procedure · To provide as appropriate consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants as their request. · To participate in the development and operation of the Clinical Directorate structure and in such management or representative structure as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures. · To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training. · To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHD’s) treating patients under the Consultant’s care. · To participate as a right and obligation in selection process for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required, The Employer shall ensure that a Consultant representative of the relevant speciality/sub-specialty is involved in the selection process. · To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures. · To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery. · To provide on-call on general Consultant On-Call rota at St. Columcille’s Hospital and share general medical on-call duties.Outside core hour’s on-call the Physician will carry out one In-Patient ward round commencing before 11am on Saturday, Sunday and Public Holidays for newly admitted patients, patients whose clinical status is deteriorating and patients identified for potential discharge and ward rounds on remaining patients. · To attend at St. Columcille’s Hospital in the National Gender Service/Endocrinology service as appropriate to the role. · To alternate with the other Consultants in the department and to act for them as required during the short term absences including weekend and off duty leave. · To provide lectures as required in the Hospital for relevant staff in training. · To treat any person referred to him/her within the National Gender Service. Managerial & Administration · To collaborate with hospital management Director Of Nursing and Medical staff of the hospital in all matters appertaining to the general efficiency of the hospital. · To furnish on request to a registered Medical Practitioner authorised by the HSE or the Minister for Health, the clinical details regarding any person who is or has being under his/her care in the hospital or clinic on the production of the written consent of the person (or the written consent of the persons representative or next of kin) · Whenever the Minister for Health is of the opinion, however, that it would not be in the interest of the common good to seek such consent and he/she certifies accordingly, the officer shall furnish the required information to a registered medical practitioner authorised by the Minister for Health. Where, however, the officer is of the opinion that the nature of the case is such that he/she should inform the patient of the requirement of the Minister for Health, he/she shall be at liberty to do so. It is not intended that this requirement will operate in a manner, which would detract from the patient’s character or reputation. · Take the lead on designated projects, initiatives or duties within the National Gender Service · Participate in the development, monitoring and review of departmental standards, protocols and guidelines to ensure optimal client care. · Ensure that the highest standards of practice appropriate to a quality service are maintained and improved within the resources available. · Devise operational policies and monitor the implementation of these policies consistent with the delivery of a high quality service. · Participate in the preparation of plans for the service and monitor and report on their implementation as required. · Participate with any internal or external evaluation of the service. · Be involved in liaising with the pharmacy to ensure adequate stocks are available, and to ensure the best quality and price of products are available for patients within the service, · Provide strategic and clinical leadership which results in the development and delivery of effective, efficient, quality assured and patient centred service. Health & Safety The Locum Consultant Endocrinologist will: · Comply with the policies, procedures and safe professional practice of the Irish Healthcare System by adhering to relevant legislation, regulations and standards. · Work in a safe manner with due care and attention to the safety of self and others. · Maintain good collaborative working relationship and communications with appropriate statutory, professional and voluntary organisations responsible for and/or participating in health care. · Monitor research and new developments. Consultant Endocrinologist (Locum) Terms and Conditions of Employment Tenure The current vacancy available is fixed term (for two years) and whole-time. The post is pensionable. A panel may be created from which permanent and specified purpose vacancies of full or part time duration may be filled. The tenure of these posts will be indicated at “expression of interest” stage. Appointment as an employee of the Health Service Executive is governed by the Health Act 2004 and the Public Service Management (Recruitment and Appointment) Act 2004. Remuneration The annual salary will be as set out in the Consultant Contract 2008 (as per standard text issued by the HSE) for POCC23 Working Week The standard working week applying to the post is 39 hours. HSE Circular 003-2009 “Matching Working Patterns to Service Needs (Extended Working Day / Week Arrangements); Framework for Implementation of Clause 30.4 of Towards 2016” applies. Under the terms of this circular, all new entrants and staff appointed to promotional posts from Dec 16th 2008 will be required to work agreed roster / on call arrangements as advised by their line manager. Contracted hours of work are liable to change between the hours of 8am-8pm over seven days to meet the requirements for extended day services in accordance with the terms of the Framework Agreement (Implementation of Clause 30.4 of Towards 2016). Annual Leave 31 days per annum Superannuation This is a pensionable position with the HSE. The successful candidate will upon appointment become a member of the appropriate pension scheme. Pension scheme membership will be notified within the contract of employment. Members of pre-existing pension schemes who transferred to the HSE on the 01st January 2005 pursuant to Section 60 of the Health Act 2004 are entitled to superannuation benefit terms under the HSE Scheme which are no less favourable to those which they were entitled to at 31st December 2004 Probation Every appointment of a person who is not already a permanent officer of the Health Service Executive or of a Local Authority shall be subject to a probationary period of 12 months as stipulated in the Department of Health Circular No.10/71.
