We support them to further their education (both internal and external to the Service) and to build their independent living skills if this is their choice. A major focus of the Support Worker role is to assist and support people who use the Service with development of their own Individual Plan and supporting them to achieve their own Personal Outcome goals. The Support Worker is also responsible for all aspects of personal care which includes supporting individuals with their meals, personal hygiene and other related aspects of physical well-being as required by the individual. Reporting/Responsible To: Team Leader/Senior Staff/Team Manager/Service Coordinator/Area Manager Qualifications/Experience/Skills: Candidates must have a good general education. A recognised qualification appropriate to the Field of intellectual disability is an advantage. QQI Level 5 or higher is desirable. A minimum of one years’ experience of working with individuals with an Intellectual Disability is desirable, full training will be provided. The candidate should have knowledge of person-centred planning. A full clean manual Driving Licence, which qualifies you to drive manual transmission vehicles on Irish Roads and use of your own car is an essential requirement. It may be necessary for the appointee to obtain a Class D1 Driving Licence (if not already in possession of one). Assistance will be given towards this purpose of obtaining a class D1 licence, should it be required. Fluency in verbal and written English is an essential requirement of this post. To be able to demonstrate flexibility, communication and team working skills together with an enthusiasm for the post. Working Hours: Full-time hours are 78 hours a fortnight/part-time hours will vary. All posts are rostered over a 14 day/night duty roster. The overarching aim of the Strategy is the development of Neuro-Rehabilitation services to improve outcomes for people by providing safe, high quality, person-centred Neuro-Rehabilitation at the lowest appropriate level of complexity. This must be integrated across the care pathway and provided as close to home as possible. These services are to be configured into population based Managed Clinical Rehabilitation Networks (MCRNs). The new model would see the introduction of a multi-tiered system, with access to services based on clinically assessed need. This means that instead of people being referred to a single service, they are now referred to a network of services and can access services within the network based on their need. All referrals to the network are received on a standardised referral form and are processed through a single point of entry through the Network Rehabilitation Coordinator role. Services would work together across organisational boundaries with people moving across the continuum of care seamlessly based on their needs. The MCRN model consists of a multi-tiered system of Neuro-Rehabilitation services and the Community Neuro-Rehabilitation Team (CNRT) provides a critical link in the care pathway by facilitating early discharge and continuity from both acute and post-acute rehabilitation facilities. The Community Neuro Rehabilitation Team is an inter-disciplinary therapy team providing a specialised service for adults with a range of progressive or acquired neurological conditions. This is a multi-professional team including a Consultant in Rehabilitation Medicine, Physiotherapy, Occupational Therapy, Speech and Language Therapy, Psychology, Social Work, Nursing and Administrative staff. This team will provide assessment and intensive therapy to people in both their own homes and in a clinic based setting for up to a period of 12 weeks. This service will provide people with earlier access to specialist rehabilitation close to home. The team provide a person centred service, improving people’s quality of life and working together to support people with neuro-rehabilitative needs. As this team is part of the Managed Clinical Rehabilitation Network, the newly established Community Neuro-rehabilitation Team will have a significant role to play with respect to evaluation of the service and contributing to further development of the team and the wider Managed Clinical Rehabilitation Network. Reporting Relationship Operational governance for this post will be through the Operational Team Lead. The professional reporting relationship for clinical governance and clinical supervision will be to the Speech and Language Therapy Manager through the professional line management structure. The above reporting relationship is currently under review and may change. Key Working Relationships The Clinical Specialist Speech and Language Therapist will work closely with the rest of the Community Neuro-Rehabilitation Team along with the team lead. The Clinical Specialist Speech and Language Therapist will link in with in-patient and community therapists as required. They will also connect with the Programme Managers for the National Neuro-Rehabilitation Strategy as required. Purpose of the Post The Speech and Language Clinical Specialist will be responsible forthe effective and efficient delivery of an evidenced based, person-centred, quality Speech and Language Therapy Service on the Community Neuro-Rehabilitation Team (CNRT). The Speech and Language Therapist, Clinical Specialist will be responsible for the provision and delivery of a high-quality, evidence-based Speech and Language Therapy Service for persons with acquired and progressive neurological conditions and will carry out clinical, educational, and administrative duties as required. The post holder, together with the interdisciplinary team will provide a specialist Neuro-Rehabilitation service. This includes providing information and advice to other professionals and carers. This may involve assisting in induction and supervision of staff, and education of Speech and Language Therapy and work experience students, when appropriate. The overarching aim of the Strategy is the development of Neuro-Rehabilitation services to improve outcomes for people by providing safe, high quality, person-centred Neuro-Rehabilitation at the lowest appropriate level of complexity. This must be integrated across the care pathway and provided as close to home as possible. These services are to be configured into population based Managed Clinical Rehabilitation Networks (MCRNs). The new model would see the introduction of a multi-tiered system, with access to services based on clinically assessed need. This means that instead of people being referred to a single service, they are now referred to a network of services and can access services within the network based on their need. All referrals to the network are received on a standardised referral form and are processed through a single point of entry through the Network Rehabilitation Coordinator role. Services would work together across organisational boundaries with people moving across the continuum of care seamlessly based on their needs. The MCRN model consists of a multi-tiered system of Neuro-Rehabilitation services and the Community Neuro-Rehabilitation Team (CNRT) provides a critical link in the care pathway by facilitating early discharge and continuity from both acute and post-acute rehabilitation facilities. The Community Neuro Rehabilitation Team is an inter-disciplinary therapy team providing a specialised service for adults with a range of progressive or acquired neurological conditions. This is a multi-professional team including a Consultant in Rehabilitation Medicine, Physiotherapy, Occupational Therapy, Speech and Language Therapy, Psychology, Social Work, Nursing and Administrative staff. This team will provide assessment and intensive therapy to people in both their own homes and in a clinic based setting for up to a period of 12 weeks. This service will provide people with earlier access to specialist rehabilitation close to home. The team provide a person centred service, improving people’s quality of life and working together to support people with neuro-rehabilitative needs. The Operational Team Manager plays a pivotal role in the efficient and effective functioning of the team. The manager holds the operational governance for the team and is the accountable and responsible person for ensuring the delivery of Community Neuro Rehabilitation Team services to the population within the defined catchment. They will co-ordinate the delivery of the services through the identification of clear access and referral pathways to services for those with neuro-rehabilitative need.
