In order to realise the benefits of Sláintecare and the future of healthcare in Ireland, there is a need for strong leadership and ownership at local level, bringing the relationship between primary care and specialist social care, mental health and acute hospital services, as well as advancing the health and wellbeing of the population, into a much more focused and integrated approach in each local area. A Community First approach to the delivery of care will be central to delivering safe, efficient and effective services. Service delivery will be re-oriented towards general practice, primary care and community-based services. By ‘shifting services left’ and prioritising Primary Care and Community Services, we will advance the goals of Sláintecare. Ownership and responsibility for the provision of health and social services, through the life cycle is best placed within the communities that people live. Community Healthcare Networks (CHNs), serving a community population of on average 50,000, are the fundamental unit of organisation for the delivery of services. A total of 96 CHNs have been identified to ensure total population coverage. It is intended that the nine Learning Sites, which commenced in 2019 as the first phase of implementation and the Enhanced Community Care Networks which were expedited in response to the COVID-19 pandemic, will facilitate the move to an increasingly integrated healthcare system which is responsive to people’s needs at the lowest level of complexity, and will ensure a focus on the population in local areas. The Therapy Manager is accountable and responsible for providing clinical governance and assurance regarding professional practice, quality and standards and will have a line of supervision to the relevant CHN/ECCN staff in this respect. They will provide a structured programme of clinical governance and assurance regarding professional practice, quality and standards working across the CHN/ECCN’s. Both the role of the CHN/ECCN Manager and the Therapy Manager are critical to the safe and effective provision of service at Network level. The Network Manager position is the key role within the overall operational management and delivery system, taking ownership of the integration of services within the Network, and with other service providers relevant to the Network. It is acknowledged that the roles of CHN/ECCN Manager and Therapy Manager are distinctive in nature. In order to enable this, the Integrated Care Programme for the Prevention and Management of Chronic Disease (ICPCD) is supporting the national implementation of a model of integrated care for the prevention and management of chronic disease as part of the Enhanced Community Care Programme (ECC). The Model of Care for the Integrated Prevention and Management of Chronic Disease has a particular focus on preventive healthcare, early intervention and the provision of supports to live well with chronic disease. The investment in the ECC programme will be delivered on a phased basis with a view to national coverage being achieved within a two- to three- year period. Three priority areas have been identified as follows: Structural reform of healthcare delivery within the community with Community Health Areas (CHAs) becoming the basic building blocks for the organisation, management and delivery of community services across the country; Creating Specialist Integrated Care Hubs within the community to support primary care management of chronic disease and older people with complex needs; and, Scaling Integrated Care for Older People and Chronic Disease through the recruitment of specialist integrated care teams including Frailty at the Front Door Teams. The ECC Programme is underpinned by a set of key principles including: Guided by the model of care for chronic disease, the NCP Diabetes aims to influence positive change and improve care for people living with diabetes across the entire spectrum of healthcare delivery: self-management support; general practice; specialist support to general practice; specialist community based integrated care hubs; and hospital inpatient specialist care. The role of the CNSp will differ according to the needs and configuration of established diabetes services at each site. They will use resources efficiently to achieve the best possible outcomes in keeping with the National Clinical Programme Diabetes model of care and HIQA standards. Alternatively provide written evidence from the Higher Education Institute that they have achieved the number of ECTS credits equivalent to a Level 9 or higher standard, relevant to the specialist area of Diabetes (equivalent to 60 ECTS or above), and in line with the requirements for specialist practice as set out by the National Council for Nursing and Midwifery 4th ed (2008). (See **Note 1 below). **Note 1: For Nurses/Midwives who express an interest in CNS/CMS roles and who currently hold a level 8 educational qualification in the specialist area (equivalent to 60 ECTS or above), this qualification will be recognised up to September 2026. AND (vii) Be required to demonstrate that they have continuing professional development (CPD) relevant to the specialist area. AND (viii) Have the ability to practice safely and effectively fulfilling his/her professional responsibility within his/her scope of practice. Annual Registration i) On appointment, practitioners must maintain live annual registration on the appropriate/relevant Division of the register of Nurses and Midwives maintained by the Nursing and Midwifery Board of Ireland (Bord Altranais agus Cnáimhseachais na hÉireann) for the role.
