Disability Services, Community Healthcare West (CHO 2) encompasses Services for Children/Young People and Adults whom have an Intellectual, Physical and Sensory Disability, Autism or Acquired Brain Injury. It also includes management/monitoring of Service Level and Grant Aid Agreements through which Section 38/39 and Private Providers are funded and development and oversight of new and emerging service delivery models. Community Healthcare West provides a broad range of services that include Primary Care, Older Persons, Disabilities, Mental Health and Health and Wellbeing. These services are delivered through the HSE and its funded agencies to people in local communities, as close as possible to people’s homes. The opportunity that will come with implementation cannot be over estimated, as it has the potential to create a far more sustainable, equitable, cost effective system and one that delivers better value for patients and service users. It creates a more sustainable opportunity to transform the health and wellbeing of the population and how and where they access services. Sláintecare sets out the need for the shift from the provision of care from acute to community settings, supporting the prevention and management of chronic disease at a community level. The strategic direction outlined in Sláintecare and the current provision of community services is underpinned by a number of strategies including Community Healthcare Organisations – Report and Recommendations of the Integrated Service Area Review Group, Healthy Ireland, Transforming Lives, The Irish National Dementia Strategy, Sharing the Vision, Connecting for Life – Ireland’s National Strategy to Reduce Suicide 2015-2020 and The National Carers’ Strategy – Recognised, Supported, Empowered. Sláintecare positions Community Healthcare Networks (CHNs) as the ‘fundamental unit of organisation for the delivery of services’ in the community. CHNs are geographically-based units delivering services to an average population of 50,000. The implementation of CHNs will see a coordinated multi-disciplinary approach to care provision, providing better outcomes for people requiring services and supports both within and across networks. New Directions is the HSE’s approach to supporting adults with disabilities who avail of HSE funded day services. New Directions is based on core values of person-centeredness, community inclusion, active citizenship and high quality service provision. It outlines twelve supports that should be available to people with disabilities participating in day services. Each person should have access to flexible and outcome driven supports to enable them to live a life of their choosing that meets their aspirations, wishes and needs. Interim standards have been developed to provide a framework to assist stakeholders to deliver day support and services in accordance with the vision and objectives outlined in New Directions. There is a national and local structure in place for the implementation of New Directions within the HSE Day service providers. Reporting Relationship The Day Service Opportunities Officerreports directly to the Day Service Opportunities Coordinator or other nominated person. Purpose of the Post The Day Opportunity Department provides people with disabilities with the appropriate guidance, support and direction concerning adult day services and other options which will enable each person to make an informed decision regarding their service requirements. The Day Opportunities Officer will meet with individuals referred to the service and explore their needs, wishes and expectations. Having a detailed knowledge of HSE funded Adult Day Services the Day Service Opportunities Officer will advise on options available and support the individual to make informed choices. If a suitable placement is agreed the Day Service Opportunities Officer will approve and complete the necessary steps required to secure the placement and ensure a smooth transition for the service participant. Identification of gaps and working with the unit to develop new and innovative service responses is also a key function of the post. This includes cross service and cross agency working to ensure the key principles of community inclusion, person centeredness and delivery of high quality services is adhered to. The Day Opportunities Officer role will also involve the completion of various resource allocation tools and other process required to secure and validate funding. This post also requires working with Day Service providers to ensure that they are providing services which meet the ongoing and changing needs of people referred and also to ensure that they are providing a high quality value for money service within their allocated budgets. Principal Duties and Responsibilities The following are the principal duties and responsibilities of the Day Opportunities Officer (Grade VII): To provide appropriate information, guidance, support and direction to enable people with disabilities and their parents/carers to make informed decisions regarding their future adult day service requirements. To participate fully with nationally agreed standardised referral process for people wishing to access HSE funded adult day services. To liaise with the appropriate personnel and assemble all information required so that an accurate assessment can be made on future service needs. Work closely with service providers to ensure that all people referred have access to the service which best meets their individual needs and wants Monitor programmes/services to ensure that they are delivered in line with the approved specifications/service arrangements and are responsive to the participants’ needs. Ensure equity of access and promote the principles of person centeredness at all times Approve funding for individual placements and ensure that approval is within the service providers annual allocations Work as part of a team in the process of agreeing annual service arrangements Work as part of a team in the monitoring and governance of non-statutory sector service arrangements To work in line with the principles of the New Directions Policy at all times and promote close working relationships between all service delivery agencies/organisations, including external agencies and community organisations. Have a detailed knowledge of the New Directions Interim Standards and support the implementation of these standards across CHO 2 To promote the development of new opportunities for people with disabilities. To work in partnership with local HSE community teams to ensure a co-ordinated delivery of services. Work closely with schools and school age teams to ensure the early identification and planning for young people leaving school requiring an adult HSE funded day service To manage case load effectively and maintain the National Day Service Database and other data management/information systems as required. Operate in line with agreed local and national process’s To demonstrate leadership and team management skills including the ability to work with multi-disciplinary team members. Incremental credit will be applied for recognised relevant service in Ireland and abroad (Department of Health Circular 2/2011). Incremental credit is normally granted on appointment, in respect of previous experience in the Civil Service, Local Authorities, Health Service and other Public Service Bodies and Statutory Agencies. Working Week The standard working week applying to the post is 35 hours HSE Circular 003-2009 “Matching Working Patterns to Service Needs (Extended Working Day / Week Arrangements); Framework for Implementation of Clause 30.4 of Towards 2016” applies. Under the terms of this circular, all new entrants and staff appointed to promotional posts from Dec 16th 2008 will be required to work agreed roster / on call arrangements as advised by their line manager. The core aim is to develop and implement specialist multidisciplinary geriatric services, with dedicated specialist in-patient wards, providing Comprehensive Geriatric Assessment (CGA), treatment, rehabilitation and appropriate discharge planning. This pathway for older persons supports better outcomes, reduced readmissions rates, reduced rates of long-term care use, reduced functional decline and improved quality of life. The senior post in Older Persons Services will support patient flow and the safe management of older persons in acute care, from the Emergency department and across to inpatient areas. The focus will be on providing early assessment and intervention for older persons and ensuring safe and timely discharge. The post will provide a 7/7 service ensuring that older persons receive the level of service required 7 days per week. The post holder will work with therapy colleagues as part of a roster.
