You will plan and implement airway clearance clinics, review clinics, oxygen assessment and pulmonary rehabilitation groups. You will work on admission avoidance pathways alongside multidisciplinary team members, GPs and respiratory consultants with excellent links to both primary and acute care. 80% of the role will be based in the Primary Care setting with 20% of the post holder’s time working with the Integrated Care Respiratory Consultant. Location of Post This is an appointment to the hub for the Integrated Care Programme for Chronic Disease, IHA KWW (Initial Base: St Mary’s Community Services, Craddockstown Road, Naas, Co. Kildare) by the Health Service Executive. The commitment for this post will be for 35 hours per week to work within the associated community-based ambulatory care hub. The 24/7 Emergency department cares for 36,000 Emergency presentations average annual activity levels of approximately 13,000 Inpatient discharges, 11,000 Day Cases Presentations and 54,000 Outpatient Attendances. This Model 3 hospital also provides medical & surgical day OPD which facilitates visiting Consultants.
