Rialto Primary Care Centre: The Nursing team consists of approximately 20 staff nurses in 11 Addiction centres across south Dublin city and the wider South Dublin area. There are also two Drug Liaison Midwives and a Hepatitis C Clinical Nurse Specialist, and a Clinical Nurse Specialist in youth drug and alcohol services. The management team consists of a Director of Nursing and two Clinical Nurse Managers. Bray Primary Care Centre: The Nursing team consists of approximately 5 staff nurses in 3 Addiction centres across Dublin South, Dun Laoghaire, and East Wicklow Community Healthcare East (DSDLEW). There is a Drug Liaison Midwife, a Hepatitis C Clinical Nurse Specialist, and an Advanced Nurse Practitioner in youth drug and alcohol services. The management team consists of a Director of Nursing and one Clinical Nurse Manager. The nursing team functions from Monday to Friday, 9am until 5pm, with some clinics providing an evening nursing service from 5pm until 8pm. Nurses may be required (after the initial orientation period) to work alone alongside the multi-disciplinary team consisting of doctors, general assistants, onsite pharmacists, counsellors, and rehabilitation integration staff. Nurses are responsible for completing initial assessments for clients who wish to start on a treatment programme. Data collected by the nurse during initial assessments are used to populate the national waiting list, Service Performance Indicators, and the National Drug Treatment Reporting System. Nurses work collaboratively with other agencies, for example mental health teams and primary care teams, to improve wellbeing and health outcomes for all clients attending the service. Nurses promote principles of self-care and empower service users to access and use health resources in their community. Nurses use bespoke care planning specific to each service user’s presenting issues. Nurses perform point of care testing, for example testing for hCG in urine specimens. The role includes venepuncture, wound management, vaccination, and responding to medical emergencies. Nurses are provided with training in line with national requirements and training specific to the addiction service, most training is provided in-service. Nurse meetings are held every month, to evaluate ongoing service needs, implement changes in line with new evidence-based practise, and distribute information.

The Pharmacy & Medicines Optimisation (PAMO) Directorate plays a pivotal role in the safe, effective and economic provision of pharmaceutical care to MMUH patients. The PAMO Directorate workload is intrinsically bound with the work of a variety of other Departments, Directorates and Hospital Management. The core services delivered within the PAMO directorate include Clinical Pharmacy, Dispensary, Drug Safety, Medicines Information and Pharmacy Cancer Services. This clinical pharmacy role primarily involves inpatient review to ensure safe, effective and economical drug use in line with MMUH policies, liaison with clinical staff to ensure appropriate drug use and drug supply / stock management for wards / departments. The Mater Hospital is Ireland’s most advanced specialist centre and is the largest university teaching hospital in the country. It provides dedicated national tertiary services for trauma, spinal surgery, cardiothoracic surgery including heart and lung transplant and is home to Ireland’s most advanced intensive care unit. The Mater Hospital ED has an annual attendance of more than 85,000 patients each year. It is recognised as the busiest adult ED in Ireland with the most diverse, high acuity and challenging patient cohort in the country. It has the most well-developed and established educational training programme in emergency and critical care with daily consultant-led training in ultrasound, simulation training, trauma and airway management and resuscitative procedures. The consultant team at The Mater Hospital ED has considerable collective international experience and post graduate qualifications in a wide range of sub specialities including pre-hospital emergency medicine, trauma care, ultrasound, sports medicine, geriatric emergency medicine as well as emergency medicine research. MDT & Patient Pathway Coordinator
Job Purpose: Purpose of the Position The post holder will work as a Patient Pathway and MDT coordinator within the organization. They will work with all members of the multi-disciplinary team to ensure that diagnosed patients are actively managed through the service pathway from point of referral, through triage highlighting any potential delays and onwards to MDT, ensuring that all national and local time to assessment and time to treatment targets are met. The post holder will be responsible for the management of the multi-disciplinary team (MDT) meetings, ensuring the meetings run effectively and are well-structured, including preparing agendas, recording attendances, ensuring required information is available, collecting data and recording agreed outcomes with the Clinical team. Active participation with the on-going establishment, development and improvement of MDTs which will expand and develop accordingly, as per service requirements.
