Job Purpose: Purpose of the Position The post holder will work as a Patient Pathway and MDT coordinator within the organization. They will work with all members of the multi-disciplinary team to ensure that diagnosed patients are actively managed through the service pathway from point of referral, through triage highlighting any potential delays and onwards to MDT, ensuring that all national and local time to assessment and time to treatment targets are met. The post holder will be responsible for the management of the multi-disciplinary team (MDT) meetings, ensuring the meetings run effectively and are well-structured, including preparing agendas, recording attendances, ensuring required information is available, collecting data and recording agreed outcomes with the Clinical team. Active participation with the on-going establishment, development and improvement of MDTs which will expand and develop accordingly, as per service requirements. ** Please read the Candidate Recruitment Pack before completing your application form ** ​​​​​​​ Main Purpose of Post / Role: The successful candidate will play a central role in delivering the Council's contribution to the Coastal Monitoring and Adaption Planning (CMAP) project, leading the design and implementation of a targeted programme to transition traditional mooring systems in Strangford Lough to advanced mooring systems (AMS). The postholder will work at the interface of marine conservation, climate adaption and community engagement, collaborating with mooring owners, stakeholders and project partners to support sustainable coastal management. This role will involce project planning and delivery, stakeholder engagement, procurement and funding coordination, and the promotion of nature-based solutions that enhance biodiversity, protect seabed habitats and build resilience to climate change within the Marine Protected Area, AONB and Geopark. This project is supported by PEACEPLUS, a programme managed by teh Special EU Programmes Body (SEUPB). Duration of Appointment: This is a temporary funded post until 31 March 2029, which may be extended or curtailed subject to funding. Salary: Scale SO1, spinal point 25-27, £36,363 - £38,220 per annum based on a 36 hour week (pro rata to hours worked). Plus 19% employer pension contribution. Pay frequency is monthly by BACS (Bank Automated Clearing System) in arrears on the third last banking day of the month. Hours of Work: Monday - Friday 36 hours per week. This post is eligible to participate in the Flexi Scheme.
