The senior community facilitator must act as a role model to person support, staff, students etc. The senior community facilitator role will contribute to service planning, ensure adherence to best practice, organisation policies and regulations, and promote inclusion, rights and wellbeing of person supported. The Senior Community facilitator supports both direct service delivery and operational duties in collaboration with the day services manager. Duties and responsibilities of any post in the Services are likely to change with the ongoing needs and developments of the Services. The Physiotherapy department provides a service to patients referred from Plastics, Reconstructive and Hand Surgery Services in West North West region. The Plastics Physiotherapy team consists of a Clinical Specialist, two Seniors and a Staff Grade. The post holder will be required to provide inpatient, outpatient and clinic cover to these patients. Plastic Surgery Clinics have recently relocated to the Merlin Park Campus and we support daily clinics there.
