Purpose of the Post The Clinical Optometrist will work as part of a multi-disciplinary team delivering a coordinated approach to client care within the Community Ophthalmic Service. The primary aim of the Primary Care Eye Services is to promote good ophthalmic health and quality of life for all categories of patients who attend the eye clinic. Patients are referred to the Community Ophthalmology Service by Public Health Nurses, School Nurses, Community Medical Doctors, General Practitioners, Ophthalmologists, Paediatricians, Optometrists and Disability Services. The Optometrists therefore have a vital role to play in providing a comprehensive and high-quality eye care service to patients in conjunction with other members of the multi-disciplinary team. The Clinical Optometrist will provide paediatric eye care. The post holder will be responsible for refractions and ocular examination of patients detecting ophthalmic pathology in patients referred to the ophthalmology service and to direct such patients for appropriate treatment. The Sligo Leitrim Primary Care Ophthalmology Service is in Sligo town and the person appointed to the post may be expected to work across the IHA. The Clinical Optometrist will be expected to participate in the implementation of the National Primary Care Eye Services Review Group Report published in 2017. Reporting Relationship The post holder will: - Report clinically/professionally to the Consultant Ophthalmologist, or delegated officer where appropriate. - Report operationally to the Business Manager, Community Ophthalmic Department. Details of Service The newly established HSE West & North West Health Region will manage and deliver all public health and social care services for Donegal, Sligo, Leitrim, Roscommon, Galway and Mayo. The redesign of services will allow new pathways to be developed between acute and community services, primary care, health and wellbeing, and voluntary sectors to create more integrated, person-centred care. HSE West and North West include all the region's community healthcare, hospitals, primary care, and public health services. Description Cork University Maternity Hospital (CUMH) is a 144 bedded Tertiary Referral Centre for Maternity, Neonatology and Gynaecology located on the campus of Cork University Hospital. CUMH incorporates the Obstetric, Gynaecology and Neonatology Services within the Health Service Executive South West region. CUMH has approximately 7, 000 births per annum and is a tertiary referral centre. Services at CUMH are delivered within the South West Region which is based on the principles of partnership and integration and strongly supports the involvement of the multidisciplinary team in service provision. CUMH is the site for clinical placements for student midwives and student nurses in undergraduate and postgraduate programmes delivered in partnership with University College Cork. CUMH Services incorporates the use of the MN-CMS (Maternal Newborn Clinical Management System) into care delivered at CUMH. The Group’s Primary Academic partner is University College Cork. A wide range of educational programmes and research is conducted within all hospitals in the Group. Six Health Regions have been established within the HSE, on the basis of the geographical boundaries agreed by the Government in July 2019 and they will be operational from 2024. Each Health Region will be tasked with population specific planning resourcing and delivery of health and social care services for the needs of its unique population. This will result in improved accountability and governance in terms of finance and performance, while also bringing decision-making closer to the frontline. Health Regions will enable and empower staff to provide services that are: • Integrated, locally planned and delivered • Easier to access and navigate • Available closer to home Health Regions are geographically-based units with clearly defined populations. They align community and hospital services within specific areas. The HSE will retain a strong but leaner central organisation, with more service provision developed at a local level. The HSE South West health region will manage and deliver all public health and social care services in Cork and Kerry. HSE South West includes all hospital and community healthcare services in the region. This includes: • South / South West Hospital Group S/SWHG • Cork Kerry Community Healthcare CKCH The Department of Population and Public Health is also now aligned with this health region Services in the South West health region: HSE Services working within this region include: • Acute Hospitals • Primary care services • Community services • Social care services • Health and social care professionals • Voluntary sector services Purpose of the post The Operational & Designated Midwife Officer will work as part of the Senior Midwifery Management Team for Maternity Services at Cork University Maternity Hospital. The post holder will provide day-to-day operational management to the services at CUMH. They will develop and lead on clinical practice in midwifery within CUMH. They will represent CUMH Maternity Services Nationally, Regionally and Locally when requested by the Director of Midwifery. The post holder is to monitor the provision of the home birth service provided under the home birth service agreement and to facilitate communication and co-operation between the SECM, expectant woman and the HSE Maternity Unit/Hospital. This will include the approval of each home birth on its merits as per the Agreement, continuous risk assessment and approval, and sign off of each birth for payment. The post holder will be responsible for all aspects of the management, monitoring and co-ordination of the HSE Home Births Service including the integration of the HSE Homebirth service within the supported care pathway. The hospital has ongoing planned outpatient infrastructure development for 2026. The services provided in the hospital include – general medicine, cardiology, respiratory, gerontology and gastro-enterology, In patient medical activity is supported by a Medical Assessment Unit and a High Dependency Unit. Mallow General Hospital also provides elective day surgery/procedures to include general, vascular ent , urology, and Endoscopy, There is an Injuries Unit, and an Outpatient department, Physiotherapy, and Radiology including CT scanning services and ultra sound scan. Specialist and Advanced Practice Nursing includes Cardiology, Respiratory, Sleep, Haemovigilance, Infection Control ,Older Adult, Tissue Viability , Obesity, Dementia and Injury Mallow General Hospital Injury Unit (IU) was established during reconfiguration of hospital services following publication of the Smaller Hospitals Framework (HSE 2013a). The IU Mallow General Hospital is one of 3 Injury Units within the Emergency Care Network (ECN) of Cork University Hospitals Group. The IU is open 8am-8pm 7 days a week. The RANP (Injury Unit) scope of clinical practice involves the management of non-life and non-limb threatening conditions, commonly known as minor injuries or ambulatory care (HSE 2012; 2013b). The RANP scope of practice directly mirrors that of the Injury Unit scope agreed at national level under the National Emergency Medicine Programme (HSE 2012). The following is an example not an exhaustive list of clinical presentations included in the scope of practice for this role. Suspected broken bones and/or dislocations to legs from knees to toes Suspected broken bones and/or dislocations to arms from collar bone (clavicle) to finger tips All sprains or strains Facial injuries/problems - minor (including oral, dental and nasal injuries) Scalds and burns - minor Wounds, bites, cuts, grazes and scalp lacerations Splinters and fish hooks Foreign bodies in eyes/ears/nose Head injury – minor (fully conscious children, who did not experience loss of consciousness or vomit after the head injury) Suturing Purpose of the post The advanced practice service is provided by nurses who practice at a higher level of capability as independent, autonomous and expert advanced practitioners. The overall purpose of the service is to provide safe, timely, evidenced based nurse-led care to patients at an advanced nursing level. This involves undertaking and documenting complete episodes of patient care, which includes comprehensively assessing, diagnosing, planning, treating and discharging patients in accordance with collaboratively agreed local policies, procedures, protocols and guidelines and/or service level agreements/ memoranda of understanding. The RANP (Injury Unit) demonstrates advanced clinical and theoretical knowledge, critical thinking, clinical leadership, and complex decision-making abilities. The RANP (Injury Unit) practices in accordance with the Code of Professional Conduct and Ethics for Registered Nurses and Registered Midwives (NMBI 2014), the Scope of Nursing and Midwifery Practice Framework (NMBI 2015), Advanced Practice (Nursing) Standards and Requirements (NMBI 2017), and the Values for Nurses and Midwives in Ireland (Department of Health 2016). The RANP (Injury Unit) service provides clinical leadership and professional scholarship in the delivery of optimal nursing services and informs the development of evidence-based health policy at local, regional and national levels. The RANP (Injury Unit) contributes to nursing research that shapes and advances nursing practice, education and health care policy at local, national and international levels.
