The majority of current staff are employed in Primary Care / Chronic Disease Management. The Midlands Community Dietetic service is managed by 1 WTE Dietitian Manager post with a current team of approx. 12 Dietitians with staff spread across Primary Care networks, Chronic Disease Management and Older Persons Services. Staff are based in HSE offices in various locations across the area including Tallaght, Armagh Road, Clane, Celbridge, Kildare Town, Heytesbury St, Thomas St, Pierce St, Rathfarnham, Ballyfermot, Clondalkin, Cherry Orchard, Ballyfermot and Inchicore and in the Midlands in Mullingar, Kilbeggan, Athlone, Longford, Portlaoise, Tullamore and Birr but other offices will be available as the service expands and with the development of Community Healthcare Networks (CHNs). Staff deliver a dietetic service to the community through individual consultations (in clinic setting or via telehealth), home visits, group education (in person or virtual) and attendance at residential care settings. With new Enhanced Community Care (ECC) investment and structure the following areas of dietetics are all getting investment of dietetic posts: •Primary Care Enhanced Community Care network posts – Adult services •Primary Care Paediatric Dietetic service •Primary Care Community Hub posts (Chronic Disease and Integrated Care Older Persons) Each CHN will deliver primary healthcare services across a population of approx. 50,000. It will consist of between 4-6 primary care teams, with GP’s involved in delivering services. Working together in multi-disciplinary teams staff will deliver the Sláintecare vision to provide the right care, in the right place at the right time. Sláintecare has given us the chance to dramatically uplift our dietetic services across the Dublin Midlands Region Adult service: With the transfer of older persons and chronic disease to the hubs there will be scope to develop and streamline adult dietetic services with a greater focus on gastroenterology service provision for example. Paediatric Service: The Primary Care Paediatric service within DSKWW is expanding with an average of 0.5WTE per network area (14 networks) and the significant development of Multidisciplinary paediatric teams (PAISTE teams). The Midlands (6 networks) is resourced at approximately 0.5 WTE Senior Dietitian Paediatrics per network. The network dietitians currently provide a general paediatric service to children within the catchment area including areas such as faltering growth, malnutrition, Non-IgE mediated Cow’s Milk Protein Allergy, GOR in infancy, Parental education on early feeding, Fussy Eating, Weight management and Coeliac disease. Dietitians provide a 1:1 service as well as group education programs (e.g. Multidisciplinary Fussy Eating program and Baby Food Made Easy) delivered in person or virtually. With increasing staff numbers we anticipate the further expansion of primary care paediatric dietetics over the coming years. Chronic Disease Hubs: (Diabetes and Weight management) The hubs operate on a hub and spoke basis with multidisciplinary teams based in the hubs but with satellite clinics and group education programmes being provided either virtually or in the networks aligned to the hub. The dietitian will work alongside a multidisciplinary team of 32.4 staff within each Hub. These new roles will play an active part in developing & delivering a dynamic progressive service and the dietitian is part of a national team of dietitians providing clinical care and Diabetes Self-Management Education and Support (DSMES) to those with a chronic disease. ICPOP: The Integrated Care for Older Persons service is a specialist multidisciplinary service primarily targeting and managing the complex care needs of the older person with multiple co-morbidities across a continuum of care. 5 new ICPOP teams are being recruited across the region The overall aims of the service are to: · Provide a specialist geriatric opinion using a multidisciplinary approach to support older people with complex care needs. · Develop a person-centered care planning approach that supports robust and timely communication across care settings. · Support appropriate and timely reduction of Emergency Department (ED) attendance through the development of care pathways that support GPs and others in assessment of older people with escalating care needs. · Provide support and education to the older person, carers and healthcare professionals.
