The Hospice is co-located on the Sligo University Hospital campus. We are seeking a temporary Palliative Medicine Consultant to join the two current Consultants in providing care across the three services which includes an on-call commitment. This temporary positon will continue until the permanent post is recruited by the Pubic Appointment Service. This job will rotate through inpatient, hospital and community services. A new single room inpatient unit will open in quarter three 2025 which will add five additional beds to the inpatient unit, the third Consultant will support the opening of the additional five beds and also support the development of ambulatory clinics in our community and hospital services. Clinical Nurse Specialist Acute Haematology Oncology

The Acute Haematology Oncology Nursing Service (AHOS) focuses on the management and treatment of patients who present acutely with cancer treatment side-effects or as an emergency complication from a known cancer diagnosis. This service was set up following the National Action Plan 2020 published by the DOH in response to COVID 19 which emphasised the need to maintain critical and ongoing services. The NCCP advocated and secured funding for clinical nurse specialist posts in all of the 26 Systemic Anti- Cancer Therapy (SACT) hospitals to safeguard patients and minimise where possible their attendance to ED and their requirement for admission to hospital. The post holder will collaborate and liaise with the NCCP on strategic goals including those set out in the National Cancer Strategy 2017 -2026 through working groups and other approaches to implementation. The Acute Haematology Oncology Nursing Service is pivotal in providing a non ED direct access route for cancer patients who require review. Cancer patients who are on treatment and become unwell can access the Clinical Nurse Specialist (Acute Haematology Oncology) via a dedicated telephone line in any of the 26 centres across the country that deliver SACT. The service provides patients with access to specialist cancer nurse advice and support during the acute phase of their SACT treatment. Unwell patients who require additional clinical review are where possible seen in the cancer day care unit or a dedicated space away from the ED. It reduces the requirement for patients to attend the ED and prioritises appropriate ED referrals. The service is well placed to link with the MDT and community services as outlined in the Sláintecare strategy.

CAMHS is a community based service, in-patient beds are sourced nationally. Each team provides a multidisciplinary approach consisting of Consultant Psychiatrists, Non Consultant Hospital Doctors, Nursing posts at various grades including, Clinical Nurse Specialists, and Advanced Nurse Practitioners. There is an Assistant Director of Nursing covering the 3 teams. Each team employs allied health professionals at various grades including Clinical Psychologists, Social Workers, Speech and Language Therapists, Occupational Therapists, Social Care Leaders, Dietetics and Administration staff. CAMHS provides services to young people 0-18 years with moderate to severe mental health illnesses including mood disorders, eating disorders, psychotic illnesses, moderate to severe anxiety disorders and neurodevelopmental disorders primarily Attention Deficit Hyperactivity Disorder (ADHD). The RANP post will support the delivery of evidence based, person centred interventions to children and young person’s attending the service. The post holder will undertake assessments, develop integrated care planning and deliver evidence based clinical interventions for a defined caseload of service users diagnosed with a mental illness. Specifically the RANP will have a key role in the management of young people with an Eating Disorder within the service. The RANP will utilise expertise and higher levels of judgement to support this service user group. The post holder will manage a clinical case load, organising pathways of care, follow up and referral procedures with primary, secondary and acute hospital services and act as an advocate in establishing, maintaining and enhancing opportunities for social inclusion at every level and in every location. The person appointed to this post will work within the Community Healthcare Organisation and work closely with the Clinical lead and multi-disciplinary teams in the acute services delivering a coordinated approach to the care of young people with eating disorders in the event of a hospital admission. This will facilitate an integrated approach to interventions for young people with an Eating Disorder and a good link between acute and community services. The RANP will work closely with any of the National Clinical Care Programmes for Mental Illness- Eating Disorder, and will support and advise on same. The RANP will organise training and monitor implementation of the agreed model of care as appropriate.
