Caredoc is seeking to employ a Physiotherapist to work as part of their multidisciplinary Community Intervention Team (CIT) Post: Physiotherapist Caredoc CIT is a specialist, multidisciplinary team which provides an acute rapid response to patients with acute illness who require enhanced care for a defined short period of time. The aims of the CIT service are to facilitate early hospital discharge and hospital attendance/admission avoidance. Care is arranged in the patient’s place of residence or in a local primary care centre as deemed appropriate. The Wexford CIT service, through its fast-tracked provision of services provides our patients with a blended model of physiotherapy, occupational therapy, and Nursing care. Referrals are accepted from the acute hospitals and General Practitioners. The CIT Physiotherapists work with patients and carers, to support the patient to remain in their own home. The focus is on maintaining or increasing independence and the ability to carry out daily living tasks. The CIT work in collaboration with colleagues across the acute, community and primary care areas. HSE Mid West (Clare / Limerick / North Tipperary) We provide services to the Community population in Clare, Limerick & North Tipperary We currently have a team of 1 Dietitian Manager and 28 Community Dietitians based throughout Clare / Limerick / North Tipperary. The Dietitians in the department provide a combination of 1 to 1 clinics and group education via face to face and virtual platforms. We provide services across multiple specialities including adults, paediatrics, home enteral nutrition, Diabetes, adult weight management and Care of the Older Persons services Each Dietitian has an office base. For example Dietitians are based in Ennis / Limerick City and Nenagh. Medical 2A is a unit comprising of 18 beds, providing care to predominantly medical patients and care of the elderly. Staffed by a CNM 2, CNM 1 and skilled nursing team the focus is to deliver safe, efficient, high-quality patient care. A strong ethos of team working exists, whereby nursing staff work closely with medical and allied health professionals to cohesively deliver patient focussed, evidence based care. Future developments will include the development of Medical 2A as the Specialist Geriatric Unit within Tipperary University Hospital.
