CUH currently employs approximately 4,571 (WTE) staff of multiple professions and is the primary teaching hospital for the Faculty of Health and Science in University College Cork (UCC). UCC is the Academic partner of the South/South West Hospital Group. CUH has very strong relationships with each of the six schools within the Science Faculty of UCC and this is a key area for future development to maximise the opportunities for both the service and academia. CUH is a Major Trauma Centre for the Republic of Ireland due to the wide range of specialties delivered by the hospital – including Neurosciences, Cardiac Services, Orthopaedics, General Surgery, Renal, Internal Medicine, Vascular, Ophthalmology, Urology, Plastic Surgery, Maxillary-Facial, Paediatrics, Intensive Care, Oncology, Haematology, Obstetrics, Gynaecology, Neonatology and Emergency Medicine. Treating patients with Oral Anticancer Medication (OAM) is a complex, multistep, multidisciplinary process, including the decision to treat, prescribing, dispensing and management of a patient on these medications. The OAM care model will see more patients move from a day-case hospital admission for treatment to a self-administered SACT model. It is estimated that the number of patients receiving oral SACT for the treatment of cancer will increase by 42-48% between 2015 and 2025 (NCCP). The population of adults with cancer that are prescribed OAM continues to grow rapidly. There are over nine thousand patients seen between physical and virtual appointments in this service annually. The OAM service will be coordinated within a nurse-led clinic. The prognosis for patients prescribed OAM is better when cared for within a specialist Oral Clinic service rather than a general day unit. As an influential member of the MDT, the CNS Oral Chemotherapy is fundamental to the delivery of this service. Clinical Nurse Specialist Palliative Care Service University Hospital
Details of Service Kerry Specialist Palliative Care Service (KSPCS) is a HSE funded service which is managed by the Head of Service of UHK who reports to the IHA lead for Kerry. Kerry Hospice Foundation is a fundraising organisation who are core to development of KSPCS working in partnership with the HSE. The Kerry Specialist Palliative Care Service (KSPCS) provided is currently led by three consultants, 24/7 on-call service which has proven of immense value to the hospital and community in improving palliative care services. The service is currently provided in the following settings: • Specialist Palliative In Patient Unit • Specialist Palliative Ambulatory Day Unit • Specialist Palliative Community Care Service • Specialist Palliative Acute Care Service Specialist Palliative Care In Patient Unit There is currently a 15 bed Specialist Palliative Care Inpatient Unit in Kerry which opened in December 2017 and it is co-located to UHK. KSPCS is currently in Phase 3 Development building works, which includes building 5 extra bedrooms. These works we hope will be completed in 2027. Patients are admitted to the Inpatient Unit for complex symptom management and end of life care that cannot be met in other care settings. Staffing of the Unit includes a CNM II, CNM Is, RGNs, Healthcare Assistants, Porters, Administration staff, Allied Health Professionals including Pharmacist, Occupational Therapists, Physiotherapists, Pastoral Care, Speech and Language Therapy, Medical Social Workers and Dietician. There is a CNM3 responsible for the IPU and Day Service. Community Specialist Palliative Care Service The Kerry Community Specialist Palliative Care Service is delivered by one team divided in 2 due to the large geographical spread of the county and its population. There are 2 bases, one in the Specialist Palliative Care Unit UHK covering North Kerry and the second base in Killarney delivering a service to South Kerry. The community nursing team includes a CNM3, Clinical Nurse Specialists (CNSs) and 2 Registered Advanced Nurse Practitioners (RANPs) for the Residential Care Facilities. In 2026, a new ANP will be recruited to join the community team. The RANPs are an integral part of KSPCS aligned to the community team and have strong integrated working relationship with Palliative Care RANP UHK. The community service delivers a seven-day service to patients in their place of residence. This may be the patient’s home, community hospital or private nursing home. The team works closely with the General Practitioners, Public Health Nursing team and staff of Residential Care Facilities. Specialist Palliative Day Ambulatory Day Unit The Specialist Palliative Care Ambulatory Day Unit is also co-located to UHK and opened in 2007. The service provides a mixed medical/social/supportive model of care. Purpose of the Post As outlined in this job description, the cCNS pathway will facilitate the post holder to be supported to professionally and clinically develop the skills and knowledge required to achieve the competencies of the CNS role. The cCNS ( Palliative Care) post holder will be enabled to deliver specialist nursing care to patients in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008. Caseload The cCNS Palliative Care will be supported to focus initially on the following patient groups: patients aged 16 years and above attending the Palliative Care service. The caseload will include patients with life limiting conditions requiring specialist palliative care. Although a very small cohort, the role may also include the provision of support and advice to paediatric patients should the need arise. The cCNS clinical role is based on the five core concepts and associated competencies for the Clinical Nurse Specialist role (adapted from NCNM 4th edition 2008).The concepts are: • Clinical Focus (Direct and Indirect Care) • Service user/Service User Advocacy • Education and Training • Audit and Research • Consultancy (including leadership in clinical practice) The Palliative Care Nursing team work closely with the Consultants and non-consultant hospital doctors who work between CUH and Marymount Hospital and part of the multidisciplinary team caring for patients with life limiting conditions. Palliative care is an approach that improves the quality of life of people facing the problems associated with life-limiting illness. The palliative care approach focuses on the prevention and relief of suffering by means of assessing and treating pain and other symptoms physical, psychological or spiritual problems. The aim of palliative care is to enhance quality of life and wherever possible to positively influence the course of illness. Palliative care also extends support to families to help them cope with their family members’ illness and their own experience of grief and loss. Purpose of the Role The post holder in collaboration with the multidisciplinary team, the Nurse Specialist is responsible for the provision of safe and effective care to the patients attending Diabetes service.
