Details of Service Within Social Care, Older Persons currently support the provision of Home Support Services to 5,000 Service Users within Dublin South and Kildare West Wicklow Community Healthcare Area. The placement of a Home Support Service is based on need assessment and seeks to support clients, carers and families within Community Care to avoid, where possible, admission to Emergency Departments and Acute Hospitals. However, where hospitalisation occurs the Home Support Service support timely, safe and appropriate homecare. The Service users are predominantly over 65 years. A request for Home Support Service reflects a multitude of referring sources from Acute and Community Services. The care support provided ranges from essential to the challengingly complex including end of life care. The care service approved is delivered by the Direct HSE Home Support and HSE commissioned Indirect Providers. The Home Support Service team locally comprises Clinical, Administration, Finance and Management Personnel. The core activities for the team are focused on managing referrals, prioritising, approving, sourcing and processing Home Support Services. The role out of the service is supported by the management of incidents/complaints and the Audit process central to the governance of the Home Support Service. The current older person’s service is reconfiguring under the Integrated Healthcare Area (IHA) direction. The Department of Health (DoH) and the Health Service Executive (HSE) has chosen interRAI as the needs assessment of choice. The implementation of interRAI is focused on Home Support in preparation for the introduction of regulations for Home Support and to meet the requirements of the upcoming Statutory Home Support scheme (SHSS) and the role out of The Health Information and Quality Authority (HIQA) regulation and standards. The positions of CNM 3 is essential to lead the clinical team to ensure delivery of quality assured care to dependant service users their families and to ensure necessary assurance in relation to the clinical governance of those services. In order to provide the necessary governance and accountability the persons appointed to these post will lead a team delivering a co-ordinated approach to the management of the Home Support Service. Purpose of the Post The Home Support Service CNM 3 role is pivotal to timely, appropriate safe service provision overseeing the highest standards within the Clinical Section of this service. Co-ordinating and engaging with referring sources where challenges arise and anticipated need for service delivery is not within the remit of the Home Support Service. Investigate, direct and respond to official requests for information where decisions taken by the Home Support Service is considered as inappropriate. Actively managing and supporting the clinical team within the service as they strive to source the most appropriate Home Support Service. Co-ordinating and managing the complexities of service provision though the Direct and Indirect commissioned care providers.
