This service will be delivered in line with the National Clinical Programme model for stroke care, aiming to ensure a seamless transition for patients who are medically fit to return home from the acute hospital setting. To support the stroke patient journey as they transition from inpatient care to community based services, ensuring coordination of care and optimisation of available resources. The post holder will also be responsible for collecting service data, to demonstrate the effectiveness of the ESD model, and will participate in service innovation initiatives in conjunction with other team members. There will be opportunities to contribute to ongoing and new service development projects, as well as research activities on site. Eligibility Criteria 1. Statutory Registration Professional Qualifications, Experience, etc (a) Candidates for appointment must: (i) Be registered on the Occupational Therapists Register maintained by the Occupational Therapists Registration Board at CORU ( https://www.coru.ie ) AND (ii) Have three years full time (or an aggregate of three years) post qualification clinical experience in clinical setting. AND (iii) Have the requisite knowledge and ability (including a high standard of suitability and professional ability) for the proper discharge of the duties of the office. AND (iv) Provide proof of Statutory Registration on the Occupational Therapists Register maintained by the Occupational Therapists Registration Board at CORU before a contract of employment can be issued. 1. Annual registration (i) On appointment, practitioners must maintain annual registration on Occupational Therapists Register maintained by the Occupational Therapists Registration Board at CORU AND (ii) Practitioners must confirm annual registration with CORU to the HSE by way of the annual Patient Safety Assurance Certificate (PSAC) 3. Clinical Research Nurse CNM
Recent years have seen astonishing and rapid advances in the understanding and treatment of cancer. Technological innovations have resulted in more rational drug design, bringing hope of better treatments to patients who have been diagnosed with advanced cancer but who may have stopped benefitting from traditional treatments such as chemotherapy. For Irish patients it is difficult however to gain access to the cutting edge of drug development and accessing new drugs is a serious issue for patients and doctors in this country. To address this, the Mater hospital has partnered with The START Centre for Cancer Care, a US physician-owned clinical trials group and one of the global leaders in early phase oncology clinical trials. Early phase (or Phase 1) trials are where new and exciting drugs are first evaluated in patients for safety and the first signs of efficacy. START Dublin is Ireland’s first and only Phase 1 clinical trial site and is physically located at and operated by The Mater Hospital. This exciting venture will meet a national need, giving our patients the opportunity to access exciting and innovative drugs that they would not otherwise be able to access in the context of a well-run and closely monitored program. Multi Disciplinary Team & Patient Pathway Coordinator
Job Purpose The post holder will work as a Patient Pathway and MDT coordinator within the organization. They will work with all members of the multi-disciplinary team to ensure that diagnosed patients are actively managed through the service pathway from point of referral, through triage highlighting any potential delays and onwards to MDT, ensuring that all national and local time to assessment and time to treatment targets are met. The post holder will be responsible for the management of the multi-disciplinary team (MDT) meetings, ensuring the meetings run effectively and are well-structured, including preparing agendas, recording attendances, ensuring required information is available, collecting data and recording agreed outcomes with the Clinical team. Active participation with the on-going establishment, development and improvement of MDTs which will expand and develop accordingly, as per service requirements.
