Curlew Adult Services provide a range of community based servicesincluding day, residential, respite, community outreach, family support and a range of multidisciplinary support services. People are supported in various types of day settings – day centres, supported employment, social and micro enterprises, training and education and these can be in groups or individualised. People are supported in residential settings in their own homes in the community individually and in groups with different staffing support levels depending on their levels of disability. Our Services are person centred focusing on the needs and abilities of adults with intellectual disabilities and autism. Our focus going forward is to form a relationship/partnership with each individual and their family and communities, supporting people to plan and direct their own service so that they can enjoy a life with a healthy balance of supports. We wish to create opportunities for people who use our service to have valued social roles in their communities and to form friendships. We are looking for people who will be flexible and adapt to the needs of particular individuals they supportand are open to change. Location: Curlew Adult Services (Boyle, Ballaghaderreen, Frenchpark Areas). Staff allocation could be either within residential/respite/day settings or in community services or in an individual’s own home. The appointee will initially commence working in one particular area of the Curlew services and this work location may change due to future service needs. The Role: Social Care Workers fulfil a key worker supportive and advocative role for individuals supported by the service. The Social Care Worker role is to support the person in all aspects of their life focusing on the individual’s ability and promoting independence and development of skills at all times. The Social Care Worker will assist and support people who use the service with development of their individual person centred plans and the achievement of associated outcomes, and critically, will support people who use the service to make informed choices and fulfil their potential. They will encourage participation in the community through inclusive and creative thinking, seek opportunities for new experiences and encourage positive risk taking in leading to informed choices. The successful candidate will provide leadership and supervision at a local level, ensuring the service delivered is person-centred. Reporting/Responsible to: Team Leader/Team Manager/Area Manager Qualifications: Candidates must hold one of the following qualifications: Experience: · Experience working with individuals with an intellectual disability and individuals with challenging behaviour is desirable. Full training will be provided. · Experience in the area of goal setting and programme implementation through a person centred approach is also desirable. · Working within a staff team and shift leading at times. Annual Leave: 22 days per annum (pro-rata for part-time), i.e. 8.46 hours per 100 hours worked. Appropriate service related leave will be granted after 5 and 10 years’ service respectively. Remuneration: Department of Health Social Care Worker salary scale as at 01/10/2024 apply: €38,951 x 11 increments - €53,970 per annum (pro-rata for part-time). A long service increment €54,989 per annum (pro-rata for part-time) is payable after 3 years on the maximum of the scale. Additional payments will be made for weekends, public holidays and night duty when worked. Salary quoted is based on a 39 hour working week. Details of the service/Background to the post In line with Sláintecare (2017) and the Department of Health’s Capacity review (2018), a shift in healthcare service provision is required to place the focus on integrated, person-centred care, based as close to home as possible. In order to enable this, the Integrated Care Programme for the Prevention and Management of Chronic Disease (ICPCD) is supporting the national implementation of a model of integrated care for the prevention and management of chronic disease as part of the Enhanced Community Care Programme (ECC). This Model of Care has a particular focus on preventive healthcare, early intervention and the provision of supports to live well with chronic disease. One of the key components of ECC has been the establishment of Integrated Care Hubs affiliated with their local hospital and serving a population of approximately 150,000. These Hubs focus primarily on the prevention and management of chronic disease, support the provision of care closer to home and facilitate ready access to diagnostics, specialist services and specialist opinions in order to enhance the delivery of patient-centred care, support early intervention and avoid hospital admission, where possible. Each hub has a suite of alternative outpatient pathways, support from multidisciplinary Chronic Disease Specialist Teams and access to diagnostics including ECHO and NT- proBNP testing, radiology, and laboratory testing as well as aConsultant Cardiologistsupporting the service to ensure the provision of the right care, in the right place, at the right time. The focus is on providing an end-to-end pathway that will reduce admissions to acute hospitals by providing access to diagnostics and specialist services in the Integrated Care Hubs in a timely manner. For patients who require hospital admission, the emphasis is on minimising the hospital length of stay, with the provision of post-discharge follow up and support for people in the community and in their own homes, where required. A shared local governance structure across the local acute hospitals and the associated Integrated Healthcare Area’s (IHA) will ensure the development of a fully integrated service and end-to-end pathway for individuals living with chronic disease. The Integrated Cardiology Service will support: · A holistic, multidisciplinary approach to the care of individuals with chronic cardiovascular disease · Provision of a reformed outpatient service that utilises telehealth and other ICT measures to facilitate a more effective and efficient delivery of care · Reduced waiting times for patients for hospital-based outpatient services · Timely access to specialist services and specialist opinion for patients with cardiovascular disease in the community · Early intervention pathways/ rapid access clinics for acute, chronic or newly presenting cardiovascular conditions · Development of pathways for the management of chronic cardiovascular disease · The early assessment and implementation of pathways that will support GP-led primary care, efficient discharge back to the community where appropriate and reduce the need for repeated hospital-based outpatient reviews · Provision of oversight and implementation of self-management support services for chronic cardiovascular disease, including cardiac rehabilitation · Facilitating access and reporting of non-invasive cardiology testing e.g. ECHO and NT-proBNP testing for GPs · Providing improved integration of early discharge, outreach and potential admission avoidance programmes The CNSp role will involve 80% working with General Practitioners (GP’s) and Community Specialist Teams (CST’s) in Integrated Care Hubs and 20% working in secondary care and / or to support the Integrated Care Consultant Cardiologist delivering care in the Integrated Care Hub. Purpose of the Post The role of the CNSp will differ according to the needs and configuration of established cardiology services at each site. The purpose of this Clinical Nurse Specialist, Cardiovascular Disease - Integrated Care post is to provide expertise and specialist nursing services to patients with a cardiovascular condition. The post holder will liaise between acute cardiology services and integrated cardiology services in the community along with other agencies to deliver effective evidenced based care. They will use resources efficiently to achieve the best possible outcomes in keeping with the ICP CD model of care and HIQA standards. The person appointed to this post will work in the Cardiology Integrated Care service. The post holder will work as part of a multidisciplinary Community Specialist Team delivering coordinated evidence based care for patients in primary care whilst liaising closely with secondary care. The CNSp (CVD-IC) will deliver nurse-led clinics to provide support to patients and their GPs in creating care management plans, assisting with diagnosis and will provide education to patients and staff. In order to ensure continuity of service to patients the CNSp may be required to rotate/cover other parts of the integrated cardiology service such as Cardiac Rehabilitation. This will also promote service integration and enhance skillset development. It will be dependent on identified service needs at each site. If deemed appropriate, site rotation should occur within the first 3-6 months of taking up the post. Job Title: : Head of Data and Analytics Transformation Vacancy ID : 092230 Vacancy Type : Permanent Post Date : 17-Jan-2025 Close Date : 31-Jan-2025 Head of Data and Analytics Transformation  PTSB is seeking applications for a newly created senior leadership role, the Head of Data and Analytics Transformation. Reporting directly to the Head of Central Data & Analytics Office (CDAO) and as a member of our bank Senior Leadership team, you will be accountable for the `Build¿ Pillar within the CDAO function, with responsibility for leading teams and capabilities across Data Architecture, Data Engineering and Data DevOps. The Head of Data & Analytics Transformation will lead the strategic design, development and optimisation of the bank¿s data infrastructure and architecture to support scalable analytics, data processing and management. This role will drive the data strategy, ensuring the efficient acquisition, integration, and provisioning of data to support business objectives across a range of initiatives. You will support strategic change initiatives by leading the delivery of modern data architecture and data engineering solutions that enable business outcomes. You will work closely with the wider CDAO senior leadership team across the 3 pillars of ¿Engage¿, ¿Build¿ and ¿Run¿. You will work in tandem with the Head of Data Operations & Services who leads the ¿Run¿ pillar in maintaining and sustaining Data Operations, Data Governance, and D&A Platforms. You will work closely with the Head of Business Partnering & Advanced Analytics who leads the ¿Engage¿ pillar to support the data and analytics needs of the enterprise. For this critical role, you will have an established track record of data-driven change delivery in a leadership role within data engineering or data architecture, also strong knowledge of ETL processes, data modelling, and data pipeline development, managing and mentoring teams to high performance and in depth knowledge of the regulatory obligations related to the success of this role. This job is a Controlled Function CF2 as defined by the Central Bank Reform Act 2010 Regulations 2011. Any appointment will be conditional on the company being satisfied that the appointee meets the requirements as set out in the Fitness and Probity standards issued by the Central Bank. This requires the company to complete prescribed due diligence to assess the appointee¿s fitness and probity. As a CF 2 role holder you will will also be subject to the Common Conduct Standards under the Central Bank of Ireland¿s Individual Accountability Framework and will be required to take reasonable steps to ensure the Conduct Standards are met. The role holder will be required to possess and maintain the appropriate technical knowledge required to perform the role and to understand the regulatory obligations to which they are subject as a CF, to include, without limitation, the Common Conduct Standards and the Fitness and Probity Standards.
