Details of Service Cork University Hospital (CUH) has approximately 800 beds and this will increase further to 1,000 beds on completion of the transfer of additional services to the CUH campus. CUH currently employs approximately 4,571 (WTE) staff of multiple professions and is the primary teaching hospital for the Faculty of Health and Science in University College Cork (UCC). UCC is the Academic partner of the South/South West Hospital Group. CUH has very strong relationships with each of the six schools within the Science Faculty of UCC and this is a key area for future development to maximise the opportunities for both the service and academia. CUH is a recommended Major Trauma Centre for the Republic of Ireland due to the wide range of specialties delivered by the hospital – including Neurosciences, Cardiac Services, Orthopaedics, General Surgery, Renal, Internal Medicine, Vascular, Ophthalmology, Urology, Plastic Surgery, Maxillary-Facial, Paediatrics, Intensive Care, Oncology, Haematology, Obstetrics, Gynaecology, Neonatology and Emergency Medicine. CUH is the tertiary referral centre for the HSE Southern area, and the supra regional area of Limerick, Clare, Tipperary, Waterford and Kilkenny. CUH therefore acts as a regional centre for secondary and tertiary care for the catchment population of approx. 550,000 served by the HSE Southern area and a supra-regional centre for a total a population of 1.2 million. Six Health Regions have been established within the HSE, on the basis of the geographical boundaries agreed by the Government in July 2019 and they will be operational from 2024. Each Health Region will be tasked with population specific planning resourcing and delivery of health and social care services for the needs of its unique population. This will result in improved accountability and governance in terms of finance and performance, while also bringing decision-making closer to the frontline. Health Regions will enable and empower staff to provide services that are: • Integrated, locally planned and delivered • Easier to access and navigate • Available closer to home Health Regions are geographically-based units with clearly defined populations. They align community and hospital services within specific areas. The HSE will retain a strong but leaner central organisation, with more service provision developed at a local level. The cCNS post holder will deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008. Caseload The cCNS will focus initially on the following service user groups: Any adult of 16 years and older requiring the Respiratory Acute Integrated Care Service. The concepts are: · Clinical Focus (Direct and Indirect Care) · Service user/client Advocacy · Education and Training · Audit and Research · Consultancy (including leadership in clinical practice) The role of the cCNS will differ according to the needs and configuration of established respiratory services at each site. The purpose of the post is to provide expertise and specialist nursing services to patients with a respiratory condition both in the ward, hospital outpatient settings and in primary care. The post holder will liaise between acute respiratory services and integrated respiratory services in the community along with other agencies to deliver effective evidenced based care. They will use resources efficiently to achieve the best possible outcomes in keeping with the NCP Programme model of care and HIQA standards. The post holder will work in the overall integrated respiratory service. This post is 50% acute based and 50% community based to work with the Consultant in Respiratory Medicine, Lead in Integrated Respiratory Care and General Internal Medicine in further development of integrated care services. The post holder will work as part of a multidisciplinary team delivering coordinated evidence based care for respiratory patients. Please note a portion of the appointees work will be carried out “offsite”. This means that the appointee will travel to the hubs to perform duties related to the role. The post holder will work with colleagues across the integrated care services to develop and implement ambulatory care pathways to improve the transition of patients between primary and secondary care. The post holder will work as part of a multidisciplinary team and in close liaison with their associated Consultant Respiratory Physician to deliver coordinated evidence based care for patients. In order to ensure continuity of service to patients the cCNS may be required to rotate/cover other parts of the integrated respiratory service such as Respiratory Integrated Care and Outreach. This will also promote service integration and enhance skillset development. It will be dependent on identified service needs at each site. If deemed appropriate, site rotation should occur within the first 3-6 months of taking up the post. The cCNS/CNS caseload will focus initially on the following patient groups. In 2019 one in seven people on the island of Ireland was aged 65 or over. The National Clinical Programme for Older People “Specialist Geriatric Services Model of Care” highlight the need to change health care practices in response to the needs of the older population. This model of care recommends that older people should have access, if required, to the following services in secondary care: · Dedicated in-patient Specialist Geriatric Wards (SGW) · Specialist Geriatric Teams (SGT): · A comprehensive Geriatric Assessment for all those identified as at risk, older persons to fully assess their individual needs and the range of services they require; · Access to in-patient rehabilitation facilities; · Ambulatory day hospital services; and · Improved links with community based services (residential care and home supports). The application of speciality focused knowledge and skills of the Clinical Nurse Specialist (CNS) Care of the Older Person (hereafter referred to the CNS) will provide for the care and on-going management of patients with conditions relating to old age. The post holder will be required to work as a key member of the older person team in the hospital providing physical, psychological and emotional support to the older person and their families/carers throughout their disease trajectory. The post holder will act as a liaison between Services for Older People in cork University Hospital, Primary Care, community services and other agencies. The appointment of the CNS is an essential element in the provision of effective, high quality, seamless integrated care for older persons with complex health care needs with a multidimensional and multidisciplinary input for acutely ill older persons as they move through the acute sector and home again. The person holding this post is required the support the principle that the care of the patient comes first at all times and will approach their work with the flexibility and enthusiasm necessary to make this principle a reality for every patient to the greatest possible degree. Details of Service As part of the new children’s hospital, there will be a new 20 bed Child and Young Person Mental Health (CYPMH) inpatient unit, to be staffed and delivered by Mental Health services within the hospital. This aligns with National Policy (VfC, MOC Paediatrics) and is a key Government, Department of Health and HSE project. The 20 bedded unit will function as two distinct pods catering for Acute/Emergency and Eating Disorder presentations, each operating as a member of a national network of such units located in Dublin, Galway, and Cork. The unit will have a regional and national remit for the treatment of the most intensive presentations in line with the CAMHS Operational Guideline. The objective of the service is to provide a cost-effective, evidence based, and outcomes focused inpatient service to improve the mental health of young people who are admitted with severe and/or complex mental health conditions. The service will; · Provide a multi-disciplinary approach to the assessment, care and treatment undertaken to meet the mental health, physical, educational/occupational, social and spiritual needs of the young person. · Provide treatment within a safe, effective, age appropriate and therapeutic environment · Involve young people in actively planning and participating in their care wherever possible and ensure effective communication and involvement of parents/ carers as appropriate · Aim to facilitate early, safe and sustainable discharge planning and support so as to ensure that young people spend the least amount of time in the inpatient unit · Collect and use feedback from young people using the service and parents/carers in the monitoring of service quality and in service development The successful candidate for this role will be a critical member of the project team, leading the design and delivery of technical and operational requirements to meet the needs of the Child and Young Person Mental Health Unit. Duration of Post There is currently two 12 month Specified Purpose Contract available – The post holder will be a member of the CYPMH Commissioning Team during the period up to the opening of the New Children’s Hospital. This team is currently led by a Clinical Project Lead and a Clinical Director. The wider Leadership team will be stood up in the coming months. It is planned that this commissioning team may transition into the managerial and clinical MDT structures to open and deliver the new services in the Child and Young Person Mental Health Unit, in the New Children’s Hospital (NCH). Clinical Nurse Specialist, Child And Adolescent Mental Health Services With Intellectual Disability

The purpose of the role is to provide support to the individual child/young person and their family and/or carer through the delivery of appropriate treatment and recovery orientated interventions in the community. In the community this will include: domiciliary visits, school visits, family support and liaison with other services. The Clinical Nurse Specialist (CAMHS-ID) will also work collaboratively with other nursing specialists areas within Cork Kerry Community Healthcare CAMHS-ID Services. This post will support the National Mental Health Clinical programme delivery. The post holder will undertake clinical assessments, initiate a range of evidenced based recovery orientated clinical interventions and link closely with a range of statutory services in secondary and primary care services and with voluntary services in the local community. They will be expected to work as a member of a mental health team, undertaking standardised assessments and management along with assertive follow up involving families/carers as appropriate.
