Our Core Values: • Collaborate • Accountability • Respect • Empathy • Trust TITLE: Home Support Workers (Subject to continued funding and successful completion of probation) DESCRIPTION OF ROLE: We require a caring, kind and considerate persons who will support Clarecare in delivering a dedicated person centred community-based Home Support Service* to people who are experiencing difficulties, thereby facilitating them to remain at home within their community. *Home Support refers to personal care and practical personal assistance with a wide range of Instrumental Activities of Daily Living (IADL’s) that can help a person live at home if they are ill, frail or disabled. It includes all forms of enabling personal care and [other] practical personal assistance provided [to] [for] an individual who by reason of illness, frailty or disability is in need of such care and personal assistance. ROLE: To provide practical Home Support (IADL’s) on behalf of Clarecare’s Home Support Service to service users to enable service users live at home in a safe secure and comfortable way. OBJECTIVE: The objective of this role is to deliver a safe, effective & efficient Home Support service in line with required practice standards, to support older people (with assessed support needs) and where relevant, their specified person(s) (e.g. family, friends who provide care) so as to enable the service user to be as independent as possible in their own home, for as long as possible. The successful candidate will work as part of the team in Clarecare and liaise regularly with same. Flexibility and willingness to travel is essential. You may be required to work outside of normal hours if necessary. Training and supervision (including shadowing where relevant) will be provided to the successful candidate. JOB RELATIONS: The Home Support Worker, will report to the Home Support Area Manager in the relevant geographical area and work as part of a wider Home Support Service team in Clarecare. Home Support Area Managers report into the Home Support Service Manager within Clarecare who has overall responsibility for Clarecare’s Home Support Service. S/he will link with other relevant Departments/Services within Clarecare as relevant and also liaise with (PHN) Public Health Nurses and other Health Support Professionals, where relevant, as part of the role. HOURS OF WORK: This is a part-time permanent role (subject to receipt of continued funding from HSE as Service Provider and successful completion of 12 months probationary period). Due to the nature of Home Support Services for Older People in the Community and HSE’s Service Provider terms of business (i.e. HSE as Service Provider offers hours for service users to Clarecare and other similar Home Support Agencies); we are unable to ascertain at any given time, the volume of business being offered/provided by HSE as Service Provider and therefore, are unable to offer regular work hours to the Home Support Worker. However, as per The Employment (Miscellaneous Provisions) Act 2018, at the end of an initial 12 months continuous service (probationary period) with Clarecare, Home Support Workers may apply to be placed on a fixed band of hours that include the average hours worked by them in the previous 12 months period. A Home Support Worker’s normal hours of work will be as outlined on the Personal Support Plan Assignment roster relevant to each Service User and hours of work provided are solely based on service user needs assessment. Amendments to assigned work hours/duties can only be made/approved by the Home Support Area Manager or designate. The Service reserves the right to alter these working hours/assigned duties from time to time based on the needs of the Service User. JOB LOCATION: The position offered is based in the community providing Home Support services to vulnerable service users in the community. The successful candidate will be required to travel as part of their daily role and may be requested from time to time, to travel to other Clarecare centre locations in the course of his/her work as and when required e.g. for training purposes, etc. Therefore, a full clean current driver’s licence, use of own vehicle for work purposes, and willingness to indemnify Clarecare re using your car for business purposes is essential for this role. Clarecare is supported by a partnership with TUSLA, Health Service Executive, other Government Departments, by charitable funding and by the Diocese of Killaloe. Clarecare Family Support Services, funded by TUSLA , offer a range of interventions to vulnerable children, including children subject to Safety Plans and/or open case to TUSLA, Child & Family Agency. We work in partnership with service users and professional colleagues in the provision of individual and group-based supports. JOB DESCRIPTION Social Care Worker – Family Support Service 1 Year Full-time, fixed term contract of employment DESCRIPTION OF ROLE: Working from the Clarecare Family Support Service practice model, which is based on the Tusla National Practice Model - Meitheal, the role of the Social Care Worker is to: • Deliver quality and innovative interventions to (a) children and young people in the community who may be the subject of child protection and welfare plans, and (b) children and young people identified as in need of support in relation to emotional and behaviour issues arising at home, school or in their community. • Deliver individual and group support interventions to parents to enhance their parenting skills and relationships with their children. • Carry out strengths and needs assessments in collaboration with children and parents and to formulate agreed outcome focussed interventions which are reviewed regularly. • Undertake the responsibilities of a Lead Practitioner in cases involved in the Tusla Meitheal process. • Actively participate in Tusla Child Protection and Welfare Conferences. This involves information gathering and report writing. • Provide targeted populations of children with opportunities to participate in community activities and facilitated groups. The groups may include; personal development groups for children, parenting groups, summer activity programmes etc. • To encourage child and youth participation in the design and quality of services which is fundamental to ensuring a child centred, rights-based approach when working with children and young people. • Work as part of a multi-disciplinary team to maximise and enhance outcomes for children. This will include regular monitoring and reviewing one’s own work and engaging in formal supervision and continuous professional development. SERVICE OBJECTIVES: • To deliver a dedicated service to children in their local community. • To improve outcomes for children through the delivery of interventions underpinned by the National Policy Framework for Children and Young People and Clarecare’s specific overarching goals that all children are (1) safe and protected from harm and are (2) achieving their full potential in learning and development. • To work in partnership with children and parents to identify their strengths and needs within their home and community environment and to work together to enhance resilience. • To provide a safe and supportive space, which will give children and parents the opportunity to express and explore issues and concerns, which they may be experiencing. • To encourage parents to build on their parenting skills and enhance relationships and communication with their children. • To link and co-operate with other disciplines and agencies in order to promote a cohesive service for children. JOB RELATIONS: The Social Care Worker will report directly to the allocated Family Support Services Team Leader and work as part of a multi-disciplinary Family Support Team. The Clarecare Family Support Services Manager has overall responsibility for Family Support Services. Reporting structures are subject to change. DAYS/HOURS OF WORK: The working week is Monday to Friday from 9.30 a.m. – 5.00 p.m. daily with 30 mins. break. These positions may involve evening or weekend work (as required) and you are required to be flexible regarding your work hours/work days. ANNUAL LEAVE ENTITLEMENT: Yearly Annual Leave entitlement is 26 days per full leave year (Jan-Dec) for this full-time, 1 year fixed term post. This entitlement excludes Public Holidays. JOB LOCATION: The office base for these positions will be Clarecare, Harmony Row, Ennis. These are on-site roles – remote/hybrid working does not apply. Specific Responsibilities • To undertake all duties as outlined in this job description for the Social Care Worker role. • To deliver a dedicated service to children and their families in their local community. This will include one to one support to children, parents and group work. • To develop and implement individual support programmes for children and young people based on models of best practice. • To work as part of the Clarecare Family Support Team in the development and running of programmes for children and families. • To develop and maintain collaborative working relationships externally with TUSLA and other relevant community, voluntary and statutory groups. • To foster and maintain positive working relationships with the local community. • To maintain up to date file records of work undertaken with the child and family and to complete reports outlining work completed. • To work as part of a team within the Family Support Team and as part of the wider Clarecare structure. • To attend relevant staff and client meetings. • To participate in local meeting fora in relation to the development of supports and services for children in the area. • To be flexible, innovative and adaptable in one’s approach to work with service users. REQUIREMENTS/QUALIFICATIONS FOR THE POSITIONS: Essential Requirements: • A nationally recognised CORU approved third level qualification to a minimum of Degree level in Social Care or Applied Social Studies (Level 7 QQI) AND • Have applied for CORU registration on the Social Care Workers Register (evidence of application will be necessary) OR • Be registered in the Social Care Workers Register maintained by the Social Care Workers Registration Board maintained by CORU. AND • Relevant experience working with disadvantaged or marginalised children/young people and/or parents in their own home, residential and community settings. • Relevant experience of working with children/young people on a one to one basis to develop and implement individual support interventions which are based on best practice and evidence informed programmes focusing on issues such as developing social skills, building resilience, coping with loss/bereavement and managing behaviour and anxiety.
