Purpose of the Post The purpose of the post is to manage the provision of a social work service within a trans /multi-disciplinary context and in so doing, ensuring a person centred service to individuals and families. Details of the Service A significant programme of reform is underway in Services for Older Persons and Chronic Disease supported by the strategic direction set out under Sláintecare (2017), the Enhanced Community Care (ECC) business case (2019), HSE Corporate Plan (2020), National Service Plan (2021) and the National Clinical Programmes. The Enhanced Community Care Reform Programme (ECC) is focused on the transformation of community care with an emphasis on establishing Community Health Networks and Specialist Community Teams working within Ambulatory Community Hubs. These plan and organise services for a defined population, enable integrated care to be implemented, shifting the focus away from acute hospitals towards a new model of specialist care in the community. The redesign of services allow new pathways to be developed between hospitals, community services, primary care, health & wellbeing and voluntary sectors to develop new networks of care for Older People and people with Chronic Disease. The Integrated Care Programme for Older Persons (ICPOP) is a specialist transdisciplinary service primarily targeting and managing the complex care needs of the older person with multiple co-morbidities across a continuum of care. The development of an Early Supported Discharge team as part of the ICPOP team aims to support and facilitate Early Supported Discharge of Older persons in the Kildare West Wicklow area The overall aims of the service are to: · Support and facilitate Early Supported Discharge of Older People with complex needs to the community setting. · Provide a specialist geriatric opinion using a trans-disciplinary approach to support older people with complex care needs. · Develop a person-centred care planning approach that supports robust and timely communication across care settings. · Support appropriate and timely reduction of Emergency Department (ED) attendance through the development of care pathways that support GPs and others in assessment of older people with escalating care needs. · Provide support and education to the older person, carers and healthcare professionals.
Human Resources Manager
Remuneration The salary scale for the post as of 01/03/2025 is: €81,444, – €82,175, – €85,389, – €88,616, – €91,818, – €95,033, – €98,231. New appointees to any grade start at the minimum point of the scale. Incremental credit will be applied for recognised relevant service in Ireland and abroad (Department of Health Circular 2/2011). Reporting Relationship The post holder will have a direct reporting relationship to the Hospital Manager, Regional Hospital Mullingar. The post holder will be professionally accountable to the Midlands Head of HR for all Human Resource management issues. Key Working Relationships The HR Manager will have key working relationships with Service Managers within RHM, Recruitment Dublin Midlands Region, Head of HR Midlands, and National HR and Health Business Services in delivery of the objectives of the role. The proper execution of duties will involve the development of appropriate communication arrangements with key stakeholders both internal and external. Purpose of the Post The HR Manager will be responsible for leading the development, delivery, and implementation of the HR function at Regional Hospital Mullingar, including management of medical workforce function. The HR Manager will work with the Hospital Manager to provide vision and strategic direction for the Regional Hospital Mullingar. Post: Temporary Consultant in Palliative Medicine Location: North West Hospice, Sligo, Co. Sligo Contract: Specified Purpose Contract for a period of 6months or until the post is filled on a permanent basis Eligibility Criteria: Relevant Consultant experience and active Specialist Registration with the Irish Medical Council is essential at time of application Service Details: The North West Hospice is an integrated specialist palliative care service comprising of an inpatient unit (7 beds), a hospital team (Sligo University Hospital) and a community based palliative care service which covers Sligo, Leitrim, South Donegal and West Cavan. The Hospice is co-located on the Sligo University Hospital campus. We are seeking a temporary Palliative Medicine Consultant to join the two current Consultants in providing care across the three services which includes an on-call commitment. This temporary positon will continue until the permanent post is recruited by the Pubic Appointment Service. This job will rotate through inpatient, hospital and community services. A new single room inpatient unit will open in quarter three 2025 which will add five additional beds to the inpatient unit, the third Consultant will support the opening of the additional five beds and also support the development of ambulatory clinics in our community and hospital services.
