Please note: Details of Service General: The Dublin South, Kildare & West Wicklow (DSKWW) Dietetic service is currently managed by 2 WTE manager posts (2 part time and 1 full time managers) with a current team of approx. 30 Dietitians. The majority of current staff are employed in Primary Care / Chronic Disease Management. The Midlands Community Dietetic service is managed by 1 WTE Dietitian Manager post with a current team of approx. 12 Dietitians with staff spread across Primary Care networks, Chronic Disease Management and Older Persons Services. Staff are based in HSE offices in various locations across the area including Tallaght, Armagh Road, Clane, Celbridge, Kildare Town, Heytesbury St, Thomas St, Pierce St, Rathfarnham, Ballyfermot, Clondalkin, Cherry Orchard, Ballyfermot and Inchicore and in the Midlands in Mullingar, Kilbeggan, Athlone, Longford, Portlaoise, Tullamore and Birr but other offices will be available as the service expands and with the development of Community Healthcare Networks (CHNs). Staff deliver a dietetic service to the community through individual consultations (in clinic setting or via telehealth), home visits, group education (in person or virtual) and attendance at residential care settings. With new Enhanced Community Care (ECC) investment and structure the following areas of dietetics are all getting investment of dietetic posts: •Primary Care Enhanced Community Care network posts – Adult services •Primary Care Paediatric Dietetic service •Primary Care Community Hub posts (Chronic Disease and Integrated Care Older Persons) Each CHN will deliver primary healthcare services across a population of approx. 50,000. It will consist of between 4-6 primary care teams, with GP’s involved in delivering services. Working together in multi-disciplinary teams staff will deliver the Sláintecare vision to provide the right care, in the right place at the right time. Sláintecare has given us the chance to dramatically uplift our dietetic services across the Dublin Midlands Region Adult service: With the transfer of older persons and chronic disease to the hubs there will be scope to develop and streamline adult dietetic services with a greater focus on gastroenterology service provision for example. Paediatric Service: The Primary Care Paediatric service within DSKWW is expanding with an average of 0.5WTE per network area (14 networks) and the significant development of Multidisciplinary paediatric teams (PAISTE teams). The Midlands (6 networks) is resourced at approximately 0.5 WTE Senior Dietitian Paediatrics per network. The network dietitians currently provide a general paediatric service to children within the catchment area including areas such as faltering growth, malnutrition, Non-IgE mediated Cow’s Milk Protein Allergy, GOR in infancy, Parental education on early feeding, Fussy Eating, Weight management and Coeliac disease. Dietitians provide a 1:1 service as well as group education programs (e.g. Multidisciplinary Fussy Eating program and Baby Food Made Easy) delivered in person or virtually. With increasing staff numbers we anticipate the further expansion of primary care paediatric dietetics over the coming years. Chronic Disease Hubs: (Diabetes and Weight management) The hubs operate on a hub and spoke basis with multidisciplinary teams based in the hubs but with satellite clinics and group education programmes being provided either virtually or in the networks aligned to the hub. The dietitian will work alongside a multidisciplinary team of 32.4 staff within each Hub. These new roles will play an active part in developing & delivering a dynamic progressive service and the dietitian is part of a national team of dietitians providing clinical care and Diabetes Self-Management Education and Support (DSMES) to those with a chronic disease. ICPOP: The Integrated Care for Older Persons service is a specialist multidisciplinary service primarily targeting and managing the complex care needs of the older person with multiple co-morbidities across a continuum of care. 5 new ICPOP teams are being recruited across the region The overall aims of the service are to: · Provide a specialist geriatric opinion using a multidisciplinary approach to support older people with complex care needs. · Develop a person-centered care planning approach that supports robust and timely communication across care settings. · Support appropriate and timely reduction of Emergency Department (ED) attendance through the development of care pathways that support GPs and others in assessment of older people with escalating care needs. · Provide support and education to the older person, carers and healthcare professionals. Specific to the Cardiac Rehabilitation To provide specialist support, we have developed a multidisciplinary model of care which incorporates a holistic assessment and individualised care pathways. We are now seeking a Clinical Nurse Specialist to provide and support existing services, and to support ongoing service development. Cardiac rehabilitation is provided primarily as an outpatient service in the Cardiac Rehabilitation Unit. There is also an outreach component as cardiac rehabilitation is delivered in community settings. Principal Duties and Responsibilities The Cardiac Rehabilitation CNS will be responsible for the co-ordination, evaluation, and development of the Cardiac Rehabilitation Service at SCH. The provision of Specialist knowledge and Clinical Expertise in the Care of Patients focusing on 5 core competencies, which are: 1. Clinical Focus. 2. Patient Advocate. 3. Education and Training. 4. Patient Advocate The CNS will: · Ensure strong patient advocacy by maintaining effective communication channels with patients, families, and all members of the multidisciplinary team · Provide an efficient, effective and high-quality service, respecting the needs of each patient. · Be aware of the existing resources /services, which help patients and their families / significant others, public health nurses and patient support groups. · Help patients and families to obtain optimal treatment considering their individual and specific needs. · Ensure appropriate and effective communication between the patient, G.P. carer and all other health care professionals involved in the patient’s care. · Review current literature and facilitate the development of information booklets for patients and families. · Present individual patient views on treatment and the provision of care to multi-disciplinary team. Education and Training Patients and their families- The CNS will: · Assess and evaluate everyone’s needs and learning abilities, taking into account experiences, attitudes and beliefs and provide educational programmes accordingly · Work closely with the multidisciplinary team in the delivery of educational sessions on disease process, for patients and their families. · Educate on lifestyle changes and on the medications, they are prescribed · Impart information, and educate patients in disease management, and lifestyle change, coping strategies etc. · Facilitate the provision of psychological and/or physiological support from other agencies as required. Staff- The CNS will: · Facilitate staff education and development by delivering education sessions for staff regarding new advancements in Cardiac Rehabilitation delivery. · Develop and deliver specific orientation and education programmes for all nurses joining the department. · Facilitate the orientation and guidance of new members of the multidisciplinary team · Engage in formal and non-formal in house education programs to provide training for staff nurses, students, and other members of the multidisciplinary team · Educate, supervise, and support junior staff to help them attain a level of competency in Cardiac Rehabilitation delivery. · Provide support, guidance, and supervision to nursing staff, ensuring integration of both clinical and theoretical practice in achieving predetermined learning objectives. Audit and Research The CNS will: · Set up clinical standards of care and ensure they are met and audited regularly. · Conduct ongoing evaluation of patient satisfaction with the clinical nurse specialist delivered education programmes. · Lead or participate in research into aspects of nursing practice in accordance with the Code of Professional Conduct for each nurse and Midwife (1998) · Be aware of research in clinical practice, set objectives and develop policies and procedures for Cardiac Rehabilitation patients, based on current international evidence-based practice · Gather information regarding effectiveness of treatments given, through judicious use of well validated and reliable audit tools. · Apply data collated to facilitate the review and revision of current practice. · Constantly update knowledge and skills and adjust clinical practice if there is sound evidence to support the change. Nurse consultant The CNS will: · Support patients and their families post diagnosis of cardiac condition. · Provide information and educate the patient and their family, to ensure they understand the nature of the condition and its implications in Phase 1/3 Cardiac Rehabilitation. · Be available to patients when adjusting to changes in their condition and provide advice and information that will enable them to make informed decisions about their care. · Be available to other nurses or health care professionals who may need advice regarding the care or treatment of Cardiac Rehabilitation patients. · Liaise with Cardiac Rehabilitation Psychologist should psychological difficulties or social issues arise and know when to refer to appropriate member of the multi-disciplinary team, i.e., social worker or psychologist. · Establish and develop clear referral guidelines and communication links within hospital departments and other hospitals in the delivery of a quality Cardiac Rehabilitation service. · Act as a resource for all health professionals in hospital and community settings. Professional Development The CNS will: · Take personal responsibility for professional growth and development. · Maintain professional development portfolio reflecting both continuous professional education and specialty specific education. · Keep abreast of the current trends and developments in professional matters. · Attend national and international meetings to maintain professional education and to facilitate feedback. · Continually re-evaluate the progress of the specialist role in the annual department service plan. Make amendments to the service based on findings and endeavour to improve the delivery of the Cardiac Rehabilitation service. Eligibility Criteria Qualifications and/ or experience Candidates must have at the latest date of application: (a) i) Be a registered nurse on the active Register of Nurses and Midwives held by An Bord Altranais agus Cnáimhseachais na hÉireann (Nursing and Midwifery Board of Ireland) or be eligible to be so registered. And ii) Be registered in the General Division of the register of Nurses and Midwives in which the application is being made. And iii) Have successfully completed a post registration programme of study, as certified by the education provider, which verifies that the applicant has achieved a Quality and Qualifications Ireland (QQI), National Framework of Qualifications (NFQ) major academic Level 8 or higher relevant to the specialist area of Cardiac Rehabilitation for e.g. Cardiovascular nursing prior to application. Or Can provide written evidence from higher education institute that they have achieved the number of ECTS credits equivalent to a Level 8 or higher standard, relevant to the specialist area Cardiac Rehabilitation nursing prior to application. And iv) Have a minimum of 5 years post registration full time experience or an aggregate of 5 years full time in the General Division of the register in which the application is being made. And v) Have a minimum of 2 years’ full time experience or an aggregate of 2 years full time experience in specialist area of cardiovascular nursing /Cardiac Rehabilitation. And vi) Demonstrate evidence of continuing professional development.
