MCRN Rehabilitation Coordinator

Applications are invited from suitably qualified candidates for the following position within the National Rehabilitation University Hospital: MCRN Rehabilitation Coordinator The role is across both - “MCRN rehabilitation coordinator and the NRH Brain Injury and Stroke Programme Rehabilitation Coordinator” (Specified Purpose of this contract is to provide backfill cover during a period of secondment Temporary, Full-time/Part-Time ) The ideal candidate would possess the following qualifications & experience on the latest date for receiving completed application forms for the office: Purpose of the Position The Rehabilitation Coordinator, as part of the NRH Brain Injury and Stroke preadmission service, will support decision making with respect to admission planning across the NRH and the HSE Managed Clinical Rehabilitation Network (MCRN) sites by ensuring that patients are directed to the most appropriate service and that admitting Consultants and admitting teams receive full and necessary clinical information on all patients to ensure a safe and effective admission for intensive rehabilitation. The role will also support co-ordination of onward referrals and seamless transfers between post-acute rehabilitation services within the MCRN. Overview of Role The rehabilitation coordinator will have a role in providing timely patient information on rehabilitation needs and readiness to the admitting consultants and the admissions group in the NRH Brain Injury and Stroke programmes with specific focus on the Managed Clinical Rehabilitation Network Demonstrator Project. The rehabilitation coordinator will work with both referring and admitting teams and services across the Network to support the safe and effective admission to the correct pathway/level of service which best meets the patients identified rehabilitation need. REQUIREMENTS: Criteria for Application: Essential · Be a Registered General Nurse or · Be a core Allied Health Professional (Therapist) · Evidence of ongoing Professional Development · 3 years post-qualification experience (excluding training) · Minimum 2 years’ experience in neurorehabilitation. 2. Desirable · Experience in field of improvement methodology and integrated care/discharge planning · Post graduate qualification applicable to the role Experience 1. Essential · Evidence of rehabilitation goal setting experience as part of IDT · Assessing IDT rehabilitation needs across all domains i.e. physical, cognitive & emotional · Knowledge and experience of research based practice · Family and patient communication/ liaison 2. Desirable · Evidence of involvement in quality/audit · Participation in Clinical Audit & Practice Development Knowledge 1. Essential · Relevant clinical knowledge related to patient populations · Sound application of theoretical knowledge in the management of acute and post-acute rehabilitation service provision · Ability to work with and motivate team · Experience/Awareness of Risk Management 2. Desirable · Evidence of rehabilitation outcome measurement and scoring e.g. FIM/FAM, RCSE etc. · Evidence of on-going CPD · Involvement in preparing and presenting written work/projects. · Background knowledge of the Neurorehabilitation Strategy & Implementation Framework Aptitudes and Skills 1. Essential · Displays leadership and skills · Strong organisational skills · Ability to work on own initiative · Ability to prioritise work and manage multiple tasks for self and others. · Ability to maintain and update database and spreadsheets 2. Desirable · Ability to compile activity data and assist with reporting according to programmatic structure and requirements · Ability to participate in relevant clinical audit related to service delivery Personal Qualities 1.Essential · Excellent interpersonal, communication, team working and leadership skills and the ability to work collaboratively · Flexibility to respond to service needs and demands · Ability to analyse complex problems, formulate solutions and implement sustainable change · Confident in working with a wide range of senior professionals We offer free parking and subsidised meals while on duty.
