Community Healthcare West provides a broad range of services that include Primary Care, Older Persons, Disabilities, Mental Health and Health and Wellbeing. These services are delivered through the HSE and its funded agencies to people in local communities, as close as possible to people’s homes. There is a national and local structure in place for the implementation of New Directions within the HSE Day service providers. Reporting Relationship The Day Service Opportunities Officerreports directly to the Day Service Opportunities Coordinator or other nominated person. Purpose of the Post The Day Opportunity Department provides people with disabilities with the appropriate guidance, support and direction concerning adult day services and other options which will enable each person to make an informed decision regarding their service requirements. The Day Opportunities Officer will meet with individuals referred to the service and explore their needs, wishes and expectations. Having a detailed knowledge of HSE funded Adult Day Services the Day Service Opportunities Officer will advise on options available and support the individual to make informed choices. If a suitable placement is agreed the Day Service Opportunities Officer will approve and complete the necessary steps required to secure the placement and ensure a smooth transition for the service participant. Having specialist clinical knowledge and skills in the area of amputee rehabilitation, the post holder will promote and demonstrate best practice and will facilitate the integration of current research theory into practice through an advanced level of clinical reasoning and decision making. He/she will act as an advanced clinical resource to Occupational Therapists and other members of the interdisciplinary team. She/he will participate in research as appropriate. The post holder will also carry a clinical caseload within the POLAR programme. Requirements: The candidate must, on the latest date for receiving completed application forms for the office, possess: · B.Sc. /Master’s Degree in Occupational Therapy approved by the Occupational Therapists Registration Board at CORU · Membership of, or eligibility for membership of the Association of Occupational Therapist of Ireland (AOTI) and CORU registered at the time of application. · A minimum of 5 years post graduate experience with a minimum of 3 years in the area of Spinal Cord Injury. · Experience of teaching and mentoring students, staff, and senior grade therapists. · Experience in clinical research.
