St. Finbarr’s Hospital is the largest community hospital in the HSE South. The hospital has 88 beds consisting of continuing care beds for the elderly, a Community Nursing Unit. The Hospital is based on a wider campus site which has many community based services. The Hospital supports the undergraduate BSc nursing programme as a placement site as well as post graduate placements for the Higher Diploma in Gerontology. There is a strong link to UCC on joint research projects in gerontological care. Six Health Regions have been established within the HSE, on the basis of the geographical boundaries agreed by the Government in July 2019 and they will be operational from 2024. Each Health Region will be tasked with population specific planning resourcing and delivery of health and social care services for the needs of its unique population. This will result in improved accountability and governance in terms of finance and performance, while also bringing decision-making closer to the frontline. Health Regions will enable and empower staff to provide services that are: • Integrated, locally planned and delivered • Easier to access and navigate • Available closer to home Health Regions are geographically based units with clearly defined populations. Physiotherapist, Clinical Specialist Pathfinder
The successful candidate will join the established Pathfinder team which is a joint initiative between the Hospital and the National Ambulance Service. This service comprises of an Advanced Paramedic (AP) and an Occupational Therapist (OT) or Physiotherapist (PT) responding to 999/112 calls from low acuity elderly patients who might otherwise present to the Hospital’s Emergency Department (ED). The team treat patients at the scene and if required, refer them to appropriate community-based services, as an alternative to ED conveyance. An additional component of the Pathfinder service is that members of the team whether OT or PT provide brief follow up intervention for patients in their home setting where appropriate. The OT and PT staff on the Pathfinder team liaise closely with each other to ensure the patient’s needs are best met by their particular skill set. The Pathfinder team work as part of a collaborative leadership team in conjunction with all relevant stakeholders. The successful candidate will be required to be present with an Advanced Paramedic in the emergency response vehicle over the course of the working week. He/she will also be required to be drive independently for follow-up intervention as required. The Clinical Specialist Physiotherapist and Senior Physiotherapist will support continuous learning and development across wider Physiotherapy Service as required. Additional Post Specific Requirements: • Demonstrate depth and breadth of experience in Physiotherapy in Care of the Older Person or Emergency Department as relevant to the role. • Demonstrate evidence of continuing professional development relevant to the area of care of the older persons/frailty in the form of post-graduate qualifications or relevant courses. • Demonstrate depth and breadth of experience in the areas of clinical audit, quality improvement initiatives, practice development, teaching and research.
