BROTHERS OF CHARITY SERVICES IRELAND – WEST REGION PERMANENT AND TEMPORARY COMMUNITY SUPPORT WORKERS FAILTE COMMUNITY SERVICES, GALWAY CITY & COUNTY JOB REF: 82452 A panel may be formed as a result of this process from which subsequent Permanent and Temporary Community Support Worker appointments may be made over the next 12 months within Fáilte Community Services. Location: Failte Community Services is a Community Outreach Programme which provides multi- disciplinary support to Adults with a Mild Intellectual Disability. Many of those we support wish to live independently in Galway City and County, to access education, work/leisure activities and community day services for young adults graduating from school or training programmes as well as those needing support with making informed choices, daily life issues, relationships and health and well-being. The service aims to promote independence through enabling adults to live a full and active life safely in their communities. This is achieved through the provision of therapeutic supports, frontline services and the promotion of meaningful community engagement. We work in on going partnership with people supported by the service and other agencies to maximise the use of community resources. We provide a services which are person centred and individualised, adaptable and versatile in order to respond to the needs of people supported by the service. All the supports are underpinned by our BOC policies and procedures, Personal Outcome Measures, Assisted Decision Making (Capacity) Act 2015 and HSE Safeguarding Vulnerable Persons at Risk of Abuse. The Role: Community Support Workers Horizon Programme provide a key-working supportive role and assist in the delivery of community support to individuals with Mild Intellectual Disability and sometimes a secondary diagnosis of Mental Health, some of whom may present with behaviours or lifestyles that can be deemed as challenging. This role comprises of mainly one to one working with individuals in the local community or sometimes in their own homes. A vital part of our Community Support Worker role is developing a community profile through; researching the local area, linking with local organisations and clubs, liaising with educational and training bodies and building positive and lasting relationships with all public and private services who assist the individuals we support to live independent lives in their communities. A major focus of the Community Support Worker role will also be encouraging and assisting individuals supported to develop their own life plans based around needs and interests. The Community Support Worker will work closely and inclusively with significant others involved in the individuals life such as family / next of kin and be supported and guided their practice by service social work, psychology, psychiatry and management when required. The Community Support Worker Arts Alive will work in our community based service in Galway City working in an exciting service with people who have a passion for life and for the arts. The team there are looking for a person who “must have a passion for supporting our Rights”, “must at all times Respect us”. They must have a passion for the arts or a willingness to support us in our passion. Established in 2010, St Luke’s Radiation Oncology Network (SLRON) operates across three sites – St Luke’s Radiation Oncology Centre at St. Luke’s Hospital (SLROC SLH), St Luke’s Radiation Oncology Centre at St James’s Hospital (SLROC SJH) and St Luke’s Radiation Oncology Centre at Beaumont Hospital (SLROC BH). The Pathology Laboratory in St. Luke’s Hospital is highly automated and computerised, providing a comprehensive analytical service to the Hospital. The service also undergoes continuous development in response to SLRON development strategy and user needs. The Blood Transfusion Department, has a key role in the therapeutic support of oncology patients during their treatment at St. Luke’s. The Pathology Laboratory comprises a small multi-disciplinary team of staff in the disciplines of: Blood Transfusion, Haematology, Biochemistry and provides a full on-call service 24/7, all year-round. Staff flexibility is important for the effective provision of a year-round laboratory service at SLRON. There is currently one permanent whole-time post available in the Blood Transfusion Department, Pathology Laboratory, St. Luke’s Radiation Oncology Centre at St. Luke’s Hospital, Rathgar (SLROC SLH). The Laboratory with its unique location within the serene and mature grounds of St. Luke’s Hospital, Rathgar, only a short distance from the city centre, offers a very attractive calm and pleasant work environment. There is ample free parking close to the lab building. The hours of operation of the Lab are 8-6pm Mon-Fri, but working times may be structured to the benefit of staff, once the overall needs of the service are met. The on-call service although 24/7, is an off-site service which is low pressure and therefore quite family-friendly. Clinical Focus The Haemovigilance Officer will: § Provide haemovigilance Service for Our Lady’s Hospital, Navan. The person employed will work in close co-operation with the Department of Blood Transfusion. § Implement guidelines for the usage of blood components developed by the Irish Blood Transfusion Services Board (IBTSB)/leading national and international bodies ie. BCSH § Promote and facilitate adherence to best practice in all aspects of Transfusion practice in particular clinical transfusion practice in wards and departments, including identification and administration of blood and blood products, and proper identification of sample § Liaise with Blood Transfusion Laboratory in investigating all reports of transfusion reactions/incidents and report adverse blood transfusion reactions to the Consultant Haematologist and the and the National Haemovigilance Office and audit of same as per the NHO guidelines § Adhere to the responsibilities of directive 2002/98/EC of the European Parliament and of the Council of 27thJanuary 2003 § Carry out post transfusion look back and product tracing and recalls § Assist in the collection of clinical and laboratory data on transfusions that can be used for formulation and updating of recommended, maximum blood order schedules (MBOS) and crossmatch/transfusion ratios § Report accidents, incidents, and complaints relating to ward/clinical activity in relation to transfusion practices to relevant authorities usually Consultant Haematologist /line manager in accordance with the laboratory quality management system § Report clinical incidents and ‘Near Miss Incidents’ to Risk Manager and SAEs/SARs to the National Haemovigilance office as per the national guidelines § Liaise closely with Blood Transfusion Laboratory staff to maintain good interdisciplinary working relationships with all staff § Develop and review SOPs to meet the requirements of ISO 15189 and the AMLBB regulations § Assist the multidisciplinary team in ensuring traceability of all blood & blood products laid down by EU Directives 2002/98/EC and 2004/23/EC § Responsibility for providing reports on Blood Component use to the hospital blood transfusion committee § Liaise with members of the Hospital Transfusion Committee and organisation of Hospital Transfusion Committee meetings § Participate in Blood Transfusion departmental meeting and attend managerial and clinical meetings that require specialist input Senior Medical Scientists or Clinical Nurse Manager II who meet the Eligibility Criteria can apply
