The Kerry Specialist Palliative Care Service (KSPCS) provided is currently lead by three consultants, 24/7 on-call service which has proven of immense value to the hospital and community in improving palliative care services. The service is currently provided in the following settings: · Specialist Palliative In Patient Unit · Specialist Palliative Ambulatory Day Service · Specialist Palliative Community Care Service · Specialist Palliative Acute Care Service Specialist Palliative Care In Patient Unit (IPU) There is a 15 bed specialist Inpatient Unit in Kerry which opened in December 2017 and is attached to UHK. Patients are admitted to the Inpatient Unit for complex symptom management, rehabilitation and end of life care that cannot be met in other settings. Staffing of the unit includes a CNM II, CNM I, RGNs, healthcare assistants, porters, clerical staff, Allied Health Professionals including occupational therapists, physiotherapists, pastoral care, pharmacist, speech and language therapist, medical social worker and dietician. Community Specialist Palliative Care Service The Kerry Community Specialist Palliative Care Service is delivered by two teams. The two teams are attributed to the large geographical spread of the county and its population. The North Kerry team is based in the Specialist Palliative Care Unit in UHK delivering a service to North Kerry and a second team is based in Killarney Community Hospital delivering a service to South Kerry. There are 2 Candidate Advanced Nurse Practitioners who work as part of the Community Nursing Team and main focus is to provide specialist service to residential care facilities Monday to Friday. It is a seven day service in the patient’s place of residence. This may be the patients home, community hospital or private nursing home. In the community, the specialist palliative care team work closely with the General Practitioner, Public Health Nurse, community hospitals and nursing homes. Specialist Palliative Day Care Service The Specialist Palliative Day Care Unit is located on the grounds of UHK and opened in 2007. The service provides a mixed medical/social/supportive model of care. Staffing of the unit includes a CNM II, RGNs, healthcare assistants, porter, administration staff, Allied Health Professionals including occupational therapist, physiotherapist, pastoral care, pharmacist, speech and language therapist, social worker and dietician. Specialist Palliative Day Care offers complimentary therapies two days a week and sessions with a psychotherapist. Music therapy sessions also available. Acute Specialist Palliative Care Service The acute specialist palliative care service includes Palliative Medical Team, one Registered Advanced Nurse Practitioner, one Clinical Nurse Specialist, one Social Worker and access to administrative support. Purpose of the Post The advanced practice service is provided by nurses who practice at a higher level of capability as independent, autonomous and expert advanced practitioners. The overall purpose of the service is to provide safe, timely, evidenced based nurse-led care to patients at an advanced nursing level .This involves undertaking and documenting complete episodes of patient care, which includes comprehensively assessing, diagnosing, planning, treating and discharging patients in accordance with collaboratively agreed local policies, procedures, protocols and guidelines and/or service level agreements/ memoranda of understanding. The Registered Advanced Nurse Practitioner – Endoscopy demonstrates advanced clinical and theoretical knowledge, critical thinking, clinical leadership and complex decision-making abilities. The Registered Advanced Nurse Practitioner – Endoscopy practices in accordance with the Code of Professional Conduct incorporating scope of practice (NMBI 2025), Advanced Practice (Nursing) Standards and Requirements (NMBI 2017), and the Values for Nurses and Midwives in Ireland (Department of Health 2016). The Registered Advanced Nurse Practitioner – Endoscopy service provides clinical leadership and professional scholarship in the delivery of optimal nursing services and informs the development of evidence based health policy at local, regional and national levels. The Registered Advanced Nurse Practitioner – Endoscopy contributes to nursing research that shapes and advances nursing practice, education and health care policy at local, national and international levels. Purpose of the Post The post of CNM 2 has a pivotal role in service planning, co-ordinating, and managing activity and resources within the clinical area. The main responsibilities are: quality assurance, resource management, staffing and staff development, practice development, facilitating communication and professional / clinical leadership. Cancer Services CUH is one of the nine designated cancer centres of the HSE National Cancer Care Programme (NCCP), one of two centres in the southern region and a designated cancer site. We provide cancer services predominantly to people who live in the counties of Cork, Kerry, South Tipperary, West Waterford and South East Limerick. Our cancer services include Rapid Access Clinic Breast, Lung, Prostate and Colorectal services; surgical cancer treatments; Medical Oncology, Radiation Oncology and Haematology Oncology consultation; chemotherapy and radiotherapy treatments; Acute pain and psycho-oncology services. Dunmanway Day Unit As one of the regional cancer centres, the Oncology Haematology Day Unit (known as the Dunmanway Day Unit) co-ordinates care of oncology /haematology patients receiving systemic anti-cancer treatment including combined modality regimes, immunotherapy , pre and post-transplant , oncology /haematology/radiotherapy trials.
