Clinical Nurse Specialist | Community Palliative Care · 1.0WTE (37.5 hours) · Indefinite duration contract · H.S.E. funded · Based in Blackrock The purpose of this post in is to provide Specialist Palliative Care (SPC) and on-going management to patients and their families/carers under the care of OLH&CS. The aim of the role is to provide for the seamless care of this patient cohort and will involve strong clinical component together with a liaison role with the multidisciplinary team (MDT), specialty services and community services as appropriate to the care of the patient and their family / carers. The CNS will work as a key member of the Consultant-led Inter-disciplinary team providing physical, psychological, social and emotional support to the patient and their family /carers as appropriate to patient need. The post holder’s practice is based on the five core concepts of the CNS role as defined by the NCNM 4th edition (2008) in order to fulfil the role. The concepts are: • Clinical Focus • Patient/Client Advocate • Education and Training • Audit and Research • Consultant Essential Requirements Qualifications: · Registered General Nurse with the Nursing & Midwifery Board of Ireland (NMBI), or eligible to register with NMBI. · Must demonstrate evidence of continuing professional development · Must have a level 8 (or higher) National Qualifications Authority of Ireland major academic award relevant to specialist palliative care. (contact Interim Head of Nursing to discuss same) · A full current driver license and access to a car Experience: · Have at minimum of 2 years post registration experience in an acute/relevant healthcare setting · A minimum of one years’ experience in specialist palliative care is desirable · Have the ability to practice safely and effectively fulfilling your professional responsibility within your scope of practice · Competent and confident IT skills - Word, Excel, Power Point and e-mail · Evidence of consistent updating of clinical skills and knowledge Desirable Requirements Qualifications: · Management Training · Teaching qualification Experience: · Experience in community nursing · Experience in policy and procedure development · Experience with quality assurance, audit and nursing research Informal enquiries for this recruitment panel advertisement are most welcome. Job Purpose: The purpose of the Medical Secretary Grade IV is to provide an exceptional and comprehensive support service delivery within the various Departments in CVRRE Directorate (Cardiology, Cardiothoracic, Vascular, Renal, Respiratory and Endocrinology) in consideration of Patient Care and the Departmental Administration Team. The Grade IV Medical Secretary will ensure that all duties, roles and responsibilities are completed to the required operational standards in line with the competency framework. Clinical Nurse Specialist Surgical Oncoprevention Service
Purpose of the Post The purpose of this Clinical Nurse Specialist, Surgical Oncoprevention Service post is: The Clinical Nurse Specialist will work with the multi-professional Breast Health team in the promotion of a patient centred approach to cancer patients and their families. In doing so, they will provide nursing expertise, education and a management consultation service to patients, their relatives and members of the multidisciplinary team in all areas of this specialty. The post holder will also be expected to contribute to nurse-led quality initiatives, which meet national, regional and local standards in cancer, supportive and palliative care. The post holder will ensure all high risk patients considering and proceeding with risk reduction surgery are supported throughout their care from surveillance, to preoperative counselling, through to surgery to post-operative discharge. Additionally it is expected that the post holder will also provide cancer care support to those high-risk gene carriers who develop cancer.
