Cork University Hospital (CUH) is a model 4, tertiary referral centre and large university teaching hospital managed by the HSE South, West Region - Integrated Health Area (IHA), Cork South and West. CUH is the tertiary referral centre for the HSE Southern Region, and the supra-regional area of Limerick, Clare, Tipperary, Waterford and Kilkenny. CUH therefore acts as a regional centre for secondary and tertiary care for the catchment population of approx 550,000 served by the HSE Southern area and a supra-regional centre for a total population of 1.1 million. The Regions academic partner is University College Cork (UCC), and it serves as one of two major trauma centres for Ireland. The hospital is one of two designated Level 1 trauma centres‡ in the country, one of two 24 hours, seven days a week neurosurgical and stroke thrombectomy centres, one of five 24 hours, seven days a week primary percutaneous coronary intervention (PPCI) centres, one of four cardiothoracic surgical centres, one of two comprehensive coagulation and haemophilia centres, and a cystic fibrosis centre. The 3 IHA in the HSE South West are Cork South and West, Cork North and East and Kerry. CUH sits within Integrated Health Area (IHA) Cork South and West. The new regional structures will provide the organisational and aligned corporate and clinical governance arrangements to enable population-based planning, management, and delivery of integrated care for people within a region. These new arrangements aim to improve the health service’s ability to deliver timely integrated care to patients and service users, care that is planned and funded in line with their needs at regional and local level. The primary objective of this reform is to help deliver universal and integrated care for people, providing them with timely access to the care they need, when they need it. This will help to ensure that the needs of people are prioritised by promoting a culture that seeks to continuously improve the access to, and quality of, health and social care services. This will be achieved by refining processes and minimising structural barriers to integrated care thereby increasing productivity and the consistency, effectiveness, and efficiency of services. Theatre Complex and Surgical Hub The Theatre Complex and Surgical Hub in CUH will ensure the provision of elective and emergency surgical procedures in the specialist theatre environments of Cardiothoracic, General, Vascular, Orthopaedic, Neurosurgery, Pancreatic, Colorectal, Plastics, Trauma, Urology, Renal, Hepatobiliary, Gynaecology and day procedures for adults and children. The Theatre Complex works on a 24/7 basis. There are a total of 13 theatres and 15 recovery bays. The Surgical Hub at Cork University Hospital (CUH) will be a dedicated facility designed to enhance day-case surgery and ambulatory capacity, addressing public waiting lists in advance of the development of regional elective hospitals. It will be located on the CUH campus, a Model 4 hospital, and will operate under the governance of CUH’s surgical/perioperative directorate. The post holder will work across all relevant areas of the CUH campus including the Theatre Complex and Surgical Hub. CUH currently employs approximately 4,571 (WTE) staff of multiple professions and is the primary teaching hospital for the Faculty of Health and Science in University College Cork (UCC). UCC is the Academic partner of the South/South West Hospital Group. CUH has very strong relationships with each of the six schools within the Science Faculty of UCC and this is a key area for future development to maximise the opportunities for both the service and academia. CUH is a Major Trauma Centre for the Republic of Ireland due to the wide range of specialties delivered by the hospital – including Neurosciences, Cardiac Services, Orthopaedics, General Surgery, Renal, Internal Medicine, Vascular, Ophthalmology, Urology, Plastic Surgery, Maxillary-Facial, Paediatrics, Intensive Care, Oncology, Haematology, Obstetrics, Gynaecology, Neonatology and Emergency Medicine. Our cancer services include Rapid Access Clinics for Breast, Lung, Prostate and Colorectal Services; Surgical Cancer Treatments; Medical Oncology, Radiation Oncology and Haematology Oncology Consultation; Chemotherapy and Radiotherapy treatments; Palliative Care and Psycho-oncology services. The Oncology Department in CUH has provided Oral Chemotherapy to patients for many years. Treating patients with Oral Anticancer Medication (OAM) is a complex, multistep, multidisciplinary process, including the decision to treat, prescribing, dispensing and management of a patient on these medications. The OAM care model will see more patients move from a day-case hospital admission for treatment to a self-administered SACT model. It is estimated that the number of patients receiving oral SACT for the treatment of cancer will increase by 42-48% between 2015 and 2025 (NCCP). The population of adults with cancer that are prescribed OAM continues to grow rapidly. There are over nine thousand patients seen between physical and virtual appointments in this service annually. The OAM service will be coordinated within a nurse-led clinic. The prognosis for patients prescribed OAM is better when cared for within a specialist Oral Clinic service rather than a general day unit. As an influential member of the MDT, the CNS Oral Chemotherapy is fundamental to the delivery of this service. Purpose of the Post The CNS post holder will deliver care in line with the five core concepts of the role set out in the Framework for the Establishment of Clinical Nurse Specialist Posts, 4th edition, National Council for the Professional Development of Nursing and Midwifery (NCNM) 2008. Caseload The CNS will focus initially on the following service user groups: Adults aged 16 and over. Purpose of the Post The CNM3 Cancer Services will help provide seamless care for all patients who access the service. The post holder plays a pivotal role within a multidisciplinary team and plays a central role in the management of activities, planning, co-ordinating and evaluation of cancer services for patients and their families and is responsible for contributing to the maintenance of the cancer database within the hospital. The CNM3 Cancer Services will communicate, negotiate and represent the patient's values with other professionals within the multidisciplinary team e.g. GPs and Public Health Nurses and will help patients obtain optimal treatment taking into account their special needs. They will attend multidisciplinary meetings, where appropriate, and ensure that patient treatment plans are initiated and implemented in a seamless manner. The post holder will have a pivotal role in service planning, development, implementation, co-coordination and management of activity and resources within the Cancer Service. The CNM 3‘s responsibilities include: overseeing the quality of nursing care, the development and implementation of policy and procedures, the monitoring of activity and the delivery of agreed levels of service for the designated areas. The CNM3 plays a key role in providing clinical and professional leadership and promoting the values of the organisation, the National Cancer Control Programme (NCCP) and HIQA standards.
