Details of the Service: As part of the 'National Strategy and Policy for the Provision of Neuro-Rehabilitation Services in Ireland – Implementation Framework 2019-2021, the HSE are developing networks of Neuro-Rehabilitation Services. The overarching aim of the Strategy is the development of Neuro-Rehabilitation services to improve outcomes for people by providing safe, high quality, person-centred Neuro-Rehabilitation at the lowest appropriate level of complexity. This must be integrated across the care pathway and provided as close to home as possible. These services are to be configured into population based Managed Clinical Rehabilitation Networks (MCRNs). The new model would see the introduction of a multi-tiered system, with access to services based on clinically assessed need. This means that instead of people being referred to a single service, they are now referred to a network of services and can access services within the network based on their need. All referrals to the network are received on a standardised referral form and are processed through a single point of entry through the Network Rehabilitation Coordinator role. Services would work together across organisational boundaries with people moving across the continuum of care seamlessly based on their needs. The MCRN model consists of a multi-tiered system of Neuro-Rehabilitation services and the Community Neuro-Rehabilitation Team (CNRT) provides a critical link in the care pathway by facilitating early discharge and continuity from both acute and post-acute rehabilitation facilities. The CNRT is an inter-disciplinary therapy team providing a specialised service for adults with a range of progressive or acquired neurological conditions. This is a multi-professional team including a Consultant in Rehabilitation Medicine, Physiotherapy, Occupational Therapy, Speech and Language Therapy, Psychology, Social Work, Nursing and Administrative staff. This team will provide assessment and intensive therapy to people in both their own homes and in a clinic based setting for up to a period of 12 weeks. This service will provide people with earlier access to specialist rehabilitation close to home. The team provide a person centred service, improving people’s quality of life and working together to support people with neuro-rehabilitative needs. As this team is part of the Managed Clinical Rehabilitation Network, the newly established Community Neuro-rehabilitation Team will have a significant role to play with respect to evaluation of the service and contributing to further development of the team and the wider Managed Clinical Rehabilitation Network. Details of Service Cork University Hospital (CUH) has approximately 800 beds and this will increase further to 1,000 beds on completion of the transfer of additional services to the CUH campus. CUH currently employs approximately 4,571 (WTE) staff of multiple professions and is the primary teaching hospital for the Faculty of Health and Science in University College Cork (UCC). UCC is the Academic partner of the South/South West Hospital Group. CUH has very strong relationships with each of the six schools within the Science Faculty of UCC and this is a key area for future development to maximise the opportunities for both the service and academia. CUH is a recommended Major Trauma Centre for the Republic of Ireland due to the wide range of specialties delivered by the hospital – including Neurosciences, Cardiac Services, Orthopaedics, General Surgery, Renal, Internal Medicine, Vascular, Ophthalmology, Urology, Plastic Surgery, Maxillary-Facial, Paediatrics, Intensive Care, Oncology, Haematology, Obstetrics, Gynaecology, Neonatology and Emergency Medicine. CUH is JAG accredited and is a Magnet4Europe Hospital. CUH is the tertiary referral centre for the HSE Southern area, and the supra regional area of Limerick, Clare, Tipperary, Waterford and Kilkenny. Physiotherapist Clinical Specialist, University Maternity Hospital
Details of the Service Cork University Hospital (CUH) is the largest single multi-speciality hospital in Ireland. It is the only Level 1 Trauma Hospital in Ireland. Regional specialities include neurosurgery, plastics and burns, cystic fibrosis and cardiac surgery. It is the centre in Cork City and county for trauma orthopaedics. The Physiotherapy Department provides physiotherapy care within Cork University Hospital, St. Finbarr’s Hospital Rehabilitation Unit and Cork University Maternity Hospital. The service also provides weekend and on-call services respiratory and orthopaedic services under specific criteria. The postholder will be required to participate in this. Flexibility will be required with regard to the working week as the post holder may be required to give classes/cover clinics in the evening and at weekends. Cork University Maternity (CUMH) Hospital opened in 2007 and involved the amalgamation of maternity services from Erinville Hospital, St. Finbarr’s Maternity Hospital, Bon Secours Maternity Unit and gynaecology services from Cork University Hospital. The CUMH physiotherapy service provides an in- and outpatient service to the specialties of obstetrics, gynaecology and neonataology in CUMH as well as Colorectal and Urology in CUH. Currently working in the service, there is a Deputy Manager, Clinical Specialists for Mesh complications, Endometriosis and for Postnatal Hub, Seniors Physiotherapists and Staff Grades. This post is primarily to support the provision of service to patients with pelvic floor dysfunction. Purpose of the Post The initial post to be offered from this panel is to be part of the development of a new multidisciplinary team in the super regional Endometriosis service in CUMH. This will primarily involve the assessment and treatment of women who have a confirmed diagnosis of Endometriosis or who have been referred to the CUMH gynaecology service for assessment. This may require the successful candidate to work in multiple off-site locations to support this. They will also be expected to further develop policies and procedures around physiotherapy care of women in order to enhance the service delivered. This involves undertaking and documenting a complete episode of patient care (assess, diagnose, plan, treat and discharge patients) according to collaboratively agreed protocols and scope of practice in the clinical setting demonstrating advanced clinical and theoretical knowledge, critical thinking, clinical leadership and decision making skills. Details of Service Bantry General Hospital (BGH) is a model 2 rural and remote hospital serving the population of West Cork and South Kerry. BGH is the only model 2 hospital nationally receiving 999 ambulance calls and accepting undifferentiated medical patients. This is due to the large rural catchment area of the hospital and long distances to the nearest model 3 or model 4 hospital. Inpatient bed capacity: BGH delivers elective day case Surgical and Endoscopy services. Other Services provided: General medicine, Gerontology, Endocrinology, Gastroenterology, Dermatology and Palliative Care and a Medical Assessment Unit, which is the admission pathway for the medical patients, along with and a High Dependency Unit to support inpatient medical activity (4 bed). Injuries Unit, Outpatients Department, Physiotherapy, and Diagnostic services Bantry General also incorporates the unique position of a HIQA Registered residential Unit within the governance of the Hospital Specialist nursing includes: ANP Acute Medicine, Diabetes, & Dementia. The purpose of these medical units is to provide timely assessment, diagnosis, and early specialised management of a wide range of medical conditions. With the overall aim of decreasing overcrowding in Emergency Departments, reducing admission rates, and decreasing lengths of stay. The Advanced Nurse Practitioner (ANP) service in MAU aims to provide safe, timely, evidence-based nurse led care. Which focuses on: · The management of lower acuity patients · Management and development of the ambulatory care pathways within the Medical Assessment Unit (MAU). · Providing continuity of care for patients and referrers to the MAU · Supporting effective patient flow within the MAU · Supporting timely discharge or admission · Provides a seamless service for patients · Improved patient outcomes through health promotion, education, and risk reduction. The NAMP recommend the improvement and standardisation of patient care through local development and use of evidence-based guidelines, algorithms and pathways. The ANP service at BGH manages and develops such pathways to facilitate ambulatory care services, improve patient flow, ensure a consistent standard of care and prevent unnecessary admissions. Ambulatory care pathways managed/developed by the ANP service to date include VTE, TIA, management of electrolyte disturbances, and chest pain. Purpose of the Post The advanced practice service is provided by nurses who practice at a higher level of capability as independent, autonomous and expert advanced practitioners. The overall purpose of the service is to provide safe, timely, evidenced based nurse-led care to patients at an advanced nursing level. This involves undertaking and documenting complete episodes of patient care, which include comprehensively assessing, diagnosing, planning, treating and discharging patients in accordance with collaboratively agreed local policies, procedures, protocols and guidelines and/or service level agreements/ memoranda of understanding. The RANP Acute Medicine demonstrates advanced clinical and theoretical knowledge, critical thinking, clinical leadership and complex decision-making abilities. Purpose of the Post The post holder will be involved in planning, developing, co-ordinating, delivering, and evaluating evidence-based, quality-assured education and training programmes for registered nurses and midwives, and other staff categories, as relevant, and as service need dictates. The successful applicant will be joining a team of Nurse Tutors where specialisation is encouraged. All Nurse Tutors will be expected to become (if not already registered) BLS Instructors and Manual Handling Instructors. If a successful applicant has a Cardiac, Critical Care or Emergency Care background, the possibility of becoming an ACLS Instructor is also an option. There will be opportunities to take a lead on Respiratory Care, Spinal Handling, Oncology, Incontinence, Stroke Management and Pain Management. Education and training programmes must be based on the most up-to-date evidence and research, and provide an engaging and challenging forum in which to foster collaborative learning, reflection on practice and innovation. The principles of quality and patient safety must underpin and be applied to all programmes. The successful candidate will have the capacity for flexibility, adaptability and willingness to respond to new challenges within a dynamic team. Advanced Nurse Practitioner, Candidate, Emergency & Acute Care, University Hospital
Details of Service Cork University Hospital (CUH) has approximately 800 beds and this will increase further to 1,000 beds on completion of the transfer of additional services to the CUH campus. CUH currently employs approximately 4,571 (WTE) staff of multiple professions and is the primary teaching hospital for the Faculty of Health and Science in University College Cork (UCC). UCC is the Academic partner of the South/South West Hospital Group. CUH has very strong relationships with each of the six schools within the Science Faculty of UCC and this is a key area for future development to maximise the opportunities for both the service and academia. CUH is a recommended Major Trauma Centre for the Republic of Ireland due to the wide range of specialties delivered by the hospital – including Neurosciences, Cardiac Services, Orthopaedics, General Surgery, Renal, Internal Medicine, Vascular, Ophthalmology, Urology, Plastic Surgery, Maxillary-Facial, Paediatrics, Intensive Care, Oncology, Haematology, Obstetrics, Gynaecology, Neonatology and Emergency Medicine. CUH is the tertiary referral centre for the HSE Southern area, and the supra regional area of Limerick, Clare, Tipperary, Waterford and Kilkenny. CUH therefore acts as a regional centre for secondary and tertiary care for the catchment population of approx. 550,000 served by the HSE Southern area and a supra-regional centre for a total a population of 1.2 million. Six Health Regions have been established within the HSE, on the basis of the geographical boundaries agreed by the Government in July 2019 and they will be operational from 2024. Each Health Region will be tasked with population specific planning resourcing and delivery of health and social care services for the needs of its unique population. This will result in improved accountability and governance in terms of finance and performance, while also bringing decision-making closer to the frontline. Purpose of the Post The main purpose of the post is to develop the job description and supporting documentation under the direction of the Health Care Provider’s Advanced Practice Stakeholder Governance Group, to enable the individual nurse to meet the NMBI Criteria for Registration as an Advanced Nurse Practitioner as set out in Advanced Practice (Nursing) Standards and Requirements (NMBI, 2017). The individual will undertake the academic preparation and develop the clinical and leadership skills, competencies and knowledge required to meet the criteria to be registered as a RANP with NMBI. The scope of the cANP role must reflect the incremental development of expertise and as such, the cANP cannot deliver care as an autonomous practitioner. The individual will complete and submit the necessary documentation for registration as an RANP with NMBI. The value of the nursing contribution as a distinct profession must be safeguarded and articulated in the development of new services led by advanced nurse practitioners, complementing rather than replacing current services delivered by doctors (NMBI, 2017 p.9). The overall purpose of the post is to provide safe, timely, evidenced based nurse-led care to patients at an advanced nursing level. This involves undertaking and documenting a complete episode of patient care (assess, diagnose, plan, treat and discharge patients) according to collaboratively agreed protocols and scope of practice in the clinical setting demonstrating advanced clinical and theoretical knowledge, critical thinking, clinical leadership and decision making skills. The advanced practice role demonstrates a high degree of knowledge, skill and experience that is applied within the nurse-patient/client relationship to achieve optimal outcomes through critical analysis, problem solving and accurate decision making (NMBI, 2017). Central to this is the provision of quality care, a safe environment and processes for patients by the use of evidence based clinical guidelines that address patient expectations, promote wellness and evaluate care given. The role will provide clinical leadership and professional scholarship in order to develop nursing practice and health policy at local, regional and national level. The role will contribute to nursing research to shape and advance nursing practice, education and health care policy at local, national and international levels.
