Provide interventions to children and their familes based on evidence based practice in relation to promoting positive behaviour and emphasing every child is an active participant in their home and local community. 2. Provide parent trainings and group work with other members of the CDNT on a regular basis in areas including but not restricted to Positive Parenting, Understanding Autism, Interacting and Playing with your child. 3. Access, plan, implement and evaluate positive behaviour support plans where required. 4. Ensure all interventions promote the rights of the child and are based on the holistic assessment of the child and their famileis needs. 5. Demonstrate a high level of specialist clinical knowledge and skills in the area of positive behaviour support to effectively carry out their duties as CNMII Behavioural Specialist. 6. Work as part of the interdisciplinary team members making and accepting referrals for children as necessary. 7. Liaise with Child and Adolescent Mental Health Teams and Child and Adolescent Mental Health of Intellectual Disability Teams. 8. Liaise and work with Respite and Home Support providers as appropriate in the adherence of Positive Behaviour Support Plans. 9. Details of Older Persons’ Service The National Integrated Care Programme for Older Persons’ (NICPOP) is leading out on the development of an end to end pathway that describes cohesive primary, secondary and acute care services for older people with a specific focus on those with more complex needs and frailty. The core aim is to develop and implement specialist multidisciplinary geriatric services, with dedicated specialist in-patient wards, providing Comprehensive Geriatric Assessment (CGA), treatment, rehabilitation and appropriate discharge planning. This pathway for older persons supports better outcomes, reduced readmissions rates, reduced rates of long-term care use, reduced functional decline and improved quality of life. The senior post in Older Persons Services will support patient flow and the safe management of older persons in acute care, from the Emergency department and across to inpatient areas. The focus will be on providing early assessment and intervention for older persons and ensuring safe and timely discharge. The post will provide a 7/7 service ensuring that older persons receive the level of service required 7 days per week. The post holder will work with therapy colleagues as part of a roster. The post holder will be primarily based on the MUH site and will be a key member of the older persons’ physiotherapy team delivering safe and effective physiotherapy care. The post holder will work with the inter-professional team within the service. The Older Persons Physiotherapy Team includes Clinical Specialist, Older Persons, Senior Physiotherapist Medical (inpatients), Senior Physiotherapist Stroke (inpatients) Senior Physiotherapist Older Persons, Senior Physiotherapist Frailty at the Front Door (FFD), staff grades and therapy assistants.
