Temporary Consultant Physician in Geriatric Medicine for 12 months The consultant geriatrician plays a central role in the identification, admission, oversight, and discharge planning for patients utilising the contracted post-acute rehabilitation beds at Teaghlach Uilinn Nursing Home. They will work closely with multidisciplinary teams (MDTs) across University Hospital Galway (UHG), Merlin Park, and community services to select suitable low-complexity rehabilitation patients and ensure they have clear, goal-oriented rehab plans. The geriatrician is responsible for confirming rehab potential, guiding referral decisions, contributing to weekly MDT reviews, and ensuring that therapy goals align with each patient’s rehabilitation progress. Once admitted, the consultant geriatrician provides ongoing clinical oversight, including participation in weekly MDT meetings and review of patients at in-person ward rounds. They support communication with GPs, community teams, and families, and contribute to discharge documentation and continuity of care. Where required, the geriatrician collaborates on escalation decisions and ensures patients’ transitions back into the community are appropriate and well-coordinated. Their leadership is key in integrating hospital, community, and nursing home care within the framework of ICPOP and Sláintecare goals. Six Health Regions have been established within the HSE, on the basis of the geographical boundaries agreed by the Government in July 2019 and they will be fully operational from 2024. Each Health Region will be tasked with population specific planning resourcing and delivery of health and social care services for the needs of its unique population. This will result in improved accountability and governance in terms of finance and performance, while also bringing decision-making closer to the frontline. Health Regions will enable and empower staff to provide services that are: · Integrated, locally planned and delivered · Easier to access and navigate · Available closer to home Health Regions are geographically-based units with clearly defined populations. They align community and hospital services within specific areas. The HSE will retain a strong but leaner central organisation, with more service provision developed at a local level. The newly established HSE South West Health Region will manage and deliver all public health and social care services for Cork & Kerry serving a population of over 700,000 people. The redesign of services will allow new pathways to be developed between acute hospitals, community services, primary care, health & wellbeing and voluntary sectors to develop more integrated, patient-centred care. The HSE South West includes all hospital and community healthcare services in the region. Details of Service Bantry General Hospital (BGH) is a 90 bed rural and remote model 2R Acute General Hospital providing inpatient, outpatient and day patient services. It provides undifferentiated medical care and day case surgical activity for the people of West Cork and South Kerry. It is a member of the South/South West Hospital Group. BGH operates on a 24 hour open access basis for undifferentiated medical intake. The hospital caters for all acute medical emergencies and has a Medical Assessment Unit and a High Dependency Unit. It also has a Rehabilitation and Acute Stroke Unit and a Care of the Elderly Ward. Bantry General Hospital provides elective Day Case Surgery, Minor Procedures and Endoscopy Day Cases. Special Nursing includes Cardiac Rehabilitation and Stroke. BGH is a site for clinical placements for the undergraduate student nurse programme delivered in partnership with the Institute of Technology, Tralee, Co. Kerry. The area provides immediate and ongoing acute medical care to presenting Adult patients in the areas of general medicine. The Medical Assessment Admissions Unit manages all referrals to the Hospital.
