Details of Service Bantry General Hospital (BGH) is a 90 bed rural and remote model 2R Acute General Hospital providing inpatient, outpatient and day patient services. It provides undifferentiated medical care and day case surgical activity for the people of West Cork and South Kerry. It is a member of the South/South West Hospital Group. BGH operates on a 24 hour open access basis for undifferentiated medical intake.The hospital caters for all acute medical emergencies and has a Medical Assessment Unit and a High Dependency Unit. It also has a Rehabilitation and Acute Stroke Unit and a Care of the Elderly Ward. Bantry General Hospital provides elective Day Case Surgery, Minor Procedures and Endoscopy Day Cases. Special Nursing includes Cardiac Rehabilitation and Stroke. BGH is a site for clinical placements for the undergraduate student nurse programme delivered in partnership with the Institute of Technology, Tralee, Co. Kerry. Six Health Regions have been established within the HSE, on the basis of the geographical boundaries agreed by the Government in July 2019 and they will be operational from 2024. Each Health Region will be tasked with population specific planning resourcing and delivery of health and social care services for the needs of its unique population. This will result in improved accountability and governance in terms of finance and performance, while also bringing decision-making closer to the frontline. Health Regions will enable and empower staff to provide services that are: • Integrated, locally planned and delivered • Easier to access and navigate • Available closer to home Health Regions are geographically-based units with clearly defined populations. They align community and hospital services within specific areas. The HSE will retain a strong but leaner central organisation, with more service provision developed at a local level. The HSE South West health region will manage and deliver all public health and social care services in Cork and Kerry. HSE South West includes all hospital and community healthcare services in the region. The primary role of the CNM 1 (HDU), in conjunction and consultation with the CNM2, will be one of clinical and professional leadership and development in the nursing team, including the development of nursing staff by means of in-service training, orientation of new staff and arranging for clinical experience and supervision of student nurses where this is appropriate. The High Dependency Unit comprises of a 4 Bedded Unit with an additional Assessment Bed, 2 of the beds are isolations bays in accordance with current infection control guidelines. The HDU accepts patients with acute respiratory, cardiac, endocrine and renal conditions. OVERALL ROLE OBJECTIVE: To carry out all material handling and inventory control activities required within a computerised warehouse environment servicing a manufacturing/ distribution/ stores operation, ensuring that stock accuracy and stock control is maintained at all times JOB SPECIFIC RESPONSIBILITIES: The post holder will: 1. Purpose of the Post The post of GP Liaison officer is a pivotal role to establish and maintain links with primary care partners and St Columcilles Hospital. Overall, this position will serve as the first point of contact by GPs who would like to refer their patients to the Medical Assessment Unit (MAU). The post holder will be expected to develop patient referral pathways for GPs to refer their patients in a timely, planned and co-ordinated fashion to the MAU, thus enhancing patient flow in SCH. In addition, the post holder will develop relationships with other primary care services to promote early patient discharge from the MAU e.g OPAT services, VHI homecare etc. Once referrals for these services are instigated in the MAU- the post holder will be required to follow up on their progress until the patient is discharged from St Columcilles. The CNM2 will work as part of the MAU multidisciplinary team to ensure the delivery of effective quality assured patient centred care. GP Liaison Officer Clinical Nurse Manager 2 Terms and Conditions of Employment Tenure The current vacancy available is Temporary and whole-time. The post is pensionable. A panel may be created from which Temporary and specified purpose vacancies of full or part time duration may be filled. The tenure of these posts will be indicated at “expression of interest” stage.
