As a Social Care Manager Grade 2 (SCM2), the post holder is required to maintain professional registration with CORU and practise in accordance with the Social Care Workers Registration Board Code of Professional Conduct and Ethics. The post holder will provide professional leadership and ensure that practice is informed by relevant legislation, regulatory requirements and recognised standards of social care practice. The post holder will provide leadership and oversight of regional transition planning processes and will be accountable for ensuring that service recommendations, pathway proposals and future service requirements are identified, developed and presented to the Regional Services Management Team to support informed decision-making, service development and resource planning. The role has overall responsibility for the planning, coordination and governance of school leaver transitions and adult service pathways, ensuring that individuals moving from school and children's services into Corlann adult services experience seamless, person-centred, rights-based and outcome-focused transitions.
