The successful candidate will join a strong administration team in a busy office, providing support to our students, academic and administrative colleagues across the School and University. Principal Duties Administrative and other duties: The duties will include but are not limited to: • General office administration, including maintenance of records, accounts, details of budgets, creation and maintenance of databases and spreadsheets, managing appointments, correspondence, maintenance of files, and circulation of information to staff; • Reception duties: answering calls, post, taking messages and responding to enquiries from prospective or past students, other staff members, and members of the public; • Support on website maintenance and management, and social media tools; • Accounts administration, i.e. processing of invoices (both manual and through the procurement system), expenses for payment and liaising with the MU Finance Office; • Working extensively with current students, in terms of communicating with students, pastoral care, and directing students to appropriate services, and other operational and student support duties that may arise in the normal course of the academic year; • Working in a collegial manner to build and maintain effective working relationships with colleagues within the School and across the University; • Assisting with the operational development and administration of timetables and examination processes; • Providing logistic support related to hosting and hospitality of various events and guest lectures held throughout the academic year; • Participating in any necessary training and development, and workshops, as relevant opportunities arise; • Learning and understanding new University procedures as they arise; • Other duties as requested from time to time by the Head of School and Administrative Lead. Healthy Age Friendly Homes Programme Local Co-ordinator

The Position Meath County Council hosts the national Age Friendly Ireland Shared Service on behalf of the local government sector. Healthy Age Friendly Homes (HAFH) is a programme run by the shared service, which has the overall objective of enabling people to remain at home (where they might otherwise have been at risk of multiple attendances and hospitalisations or premature admission to long-term nursing home care) by coordinating more integrated community-based services, providing more sustainable housing supports, supporting better connection to good public realm and providing more assistive technologies in an age friendly way. This programme is now being rolled out nationally, through all local authorities and Meath County Council invites applications from suitably qualified persons who wish to be considered for inclusion on a panel from which permanent and fixed-term contract vacancies for the position of Healthy Age Friendly Homes Local Co-ordinator shall be drawn (for Tipperary). Please note that Meath County Council may offer a position to an individual placed on the panel a position in another county in the region should a vacancy arise. Local Coordinators work within the multi-agency structure of the Age Friendly Ireland Shared Service to deliver on the objectives and actions of the HAFH programme in these areas. They are part of a team of 44 Local Coordinators across the country, operating within a structure of six regions nationally. Each Local Coordinator will report to a Regional HAFH Programme Manager and will be based in the County Council Office in their area. The HAFH Programme is overseen by a National HAFH Programme Manager. Age Friendly Programme The national Age Friendly Ireland Shared Service brings together, supports and provides technical guidance to the 31 local authority led, multi-agency Age Friendly Programmes. At local level it is developed through cooperation and partnership of older people with government agencies and service providers working side by side. Under the leadership of the local authority Chief Executives and senior managers, governance is anchored in the multi-agency age-friendly alliances, supported by broadly representative older peoples’ councils engaged as co-design partners. The Age Friendly Ireland Shared Service provides the collaborative framework to involve senior leaders from key stakeholders such as the Local Authority, HSE, An Garda Síochána, Chambers of Commerce, community sector, third-level institutions and others, who have sufficient influence and resources to develop and deliver ambitious and collaborative Age Friendly Programmes. The Programme for Government, agreed in July 2020, pledges a mission of Universal Healthcare which includes a commitment to accelerating the implementation of Sláintecare, the health reform programme and a vision of an Age Friendly Ireland. It commits to a continued investment in healthcare infrastructure in line with Project Ireland 2040. Many objectives are outlined including the expansion of primary care and ensuring the vast majority of healthcare services are available in the home or close to home. The general approach is to support older people to live in their own home with dignity and independence, for as long as possible. The Healthy Age Friendly Homes programme’s ambition is to enable everyone to live longer, healthier lives by keeping care close to home and expanding the range of health and social care services in the community. This is particularly important for older people, who access these services more frequently. It is about providing a range of alternatives for older people who may otherwise transition into long-term residential care prematurely. Sláintecare is the long-term cross-government programme to transform our health and social care services. It is the roadmap for building a world-class health and social care service for the Irish people. The Sláintecare vision is to achieve a universal single-tier health and social care system where everyone has equal access to services based on need, and not ability to pay. Over time, everyone will be entitled to a comprehensive range of primary, acute and social care services. Sláintecare aims are to improve patient and service user experience; improve clinician experience, lower costs and achieve better outcomes. Healthy Age Friendly Homes Programme The Healthy Age Friendly Homes (HAFH) Programme is an initiative, funded by the Department of Health, that aims to enable older people to continue living in their homes or in a home more suited to their needs, live with a sense of independence and autonomy, be and feel part of their community, and support the avoidance of early or premature admission to long-term residential care. The Age Friendly Ireland shared service together with Ireland’s 31 local authorities manage the HAFH Programme of support specifically aimed at the health and well-being needs of older people living in our communities. HAFH aims to support up to 10,425 older people annually. It is the aim of the programme to target supports at these older people in the form of improving their living conditions, access to continued health and social care supports and other tailored community supports. Ultimately this approach aims to: • Support the avoidance of early or premature admission to long-term residential care; • Enable older people to continue living in their homes or in a home more suited to their needs (Rightsizing); • Live with a sense of independence and autonomy; and • Be and feel part of their communities. Purpose, Duties and Responsibilities The duties of the post of the Local Coordinator include, but are not limited to: • Case manage supports for HAFH Programme participants, including carrying out home visits, conducting assessments to identify their specific needs, and agreeing a plan for their care and support needs. • Identify how the needs of older people may be served from existing providers and implement the care plan by coordinating supports across multiple agencies and groups to help ensure delivery. • Ensure, where possible, existing initiatives are utilised to support people, and as such, are synchronised in line with a support plan. • Manage their own caseload of participants, and complete regular outcome reports on the progress of the HAFH Programme to inform its evaluation. • Continually review support plans, follow up with participants as required, and assess the effectiveness of supports provided. • Establish a system for referrals to the HAFH programme, embedding new care pathways as appropriate by linking in with local stakeholders including GPs, Practice Nurses, Housing Officers and Inspectors, Local Authority based social workers, Public Health Nurses, Occupational Therapists, Discharge Planning Nurses, ICPOP and others. • Work closely with the Age Friendly HAFH Programme Manager at local level to link with the relevant personnel and stakeholders to ensure progress. • Identify and secure the support of senior managers in key agencies within the agreed geographical area to participate in the HAFH Programme including HSE, Local Authority, An Garda Síochána, voluntary and community groups and services, and all other relevant partners. • Develop a number of resources to support the delivery of the HAFH objectives, including: (i) a population and area profile of the county; (ii) an asset map to identify current resources and services and inform future planning; (iii) a robust multi-sectoral directory of all local services and supports (housing supports, health services, environmental, transport services, community and voluntary supports, physical and leisure activities, volunteer programmes, civic and social participation, education and training, technological supports, social supports and others). • Support the development of the HAFH Programme regionally and nationally in collaboration with other Local Coordinators, Regional Managers and the National Manager. • Contribute to the strategic and policy recommendations of the HAFH Programme. • Ensure that work programmes are implemented within allocated budgets. • Contribute to required reports on HAFH programme progress and effectiveness including recommendations for ‘live’ administrative changes. • Undertake any other duties of a similar level and responsibilities as may be required from time to time. • Participate in the Research and Evaluation of the HAFH programme and support the participation of stakeholders in these processes. Qualifications for the Post Character Each candidate must be of good character. Health Each candidate must be in a state of health such as would indicate a reasonable prospect of ability to render regular and efficient service. Education and Experience Each candidate must, on the latest date for receipt of completed application forms: • Have a good standard of education; and • Have not less than three years satisfactory experience in a development role; and • Possess a good standard of administrative experience and adequate experience in an office organisation and in the control of staff. Non-Irish Qualifications must be accompanied by a determination from Quality and Qualifications Ireland (QQI) to establish their comparability against the Irish National Framework of Qualifications. Overseas qualifications must also be accompanied by a translation document. The successful candidate will have: • Experience and a proven track record in person-centred care planning or service delivery or service coordination in the private, public, or voluntary sector. • Experience of working in a multi-agency or multi-stakeholder environment. • Excellent interpersonal and communication skills. Candidates must also: • Have the knowledge and ability to discharge the duties of the post concerned; • Be suitable on the grounds of character; • Be suitable in all other relevant respects for appointment to the post concerned; • Hold a full unendorsed Category B Driving Licence and access to own car. Miscellaneous Provisions Salary Salary scale: €60,011 - €78,015 per annum (EL 07/25), analogous to the Administrative Officer grade. Payment of increments is dependent on satisfactory performance. Entry point of this scale will be determined in accordance with the appropriate Department Circulars. Hours of Work The normal working hours will be 35 hours per week. Duties may require the successful applicant to work outside of normal working hours, including attendance at evening or weekend training duties, as part of normal working hours without additional remuneration. All hours worked will be subject to the provision of the Organisation of Working Time Act, 1997 as amended. You may be required to complete a time-sheet in accordance with the Organisation of Working Time Regulations, 2001. Start Date Meath County Council shall require a person to whom an offer of employment is made to take up an appointment within a reasonable period of time, usually two months from date of offer, to meet the organisational requirements of the Council. Meath County Council, as employer, must be indemnified on your insurance policy. The Role of a Decision-Making Representative A decision-making representative is a person appointed by the court to make specific decision(s) on behalf of a person (‘relevant person’) in relation to their personal welfare or property and affairs or both. When a person does not have a suitable person who can act as their decision-making representative, the court may ask the DSS to nominate two or more panel members from the Panel to be appointed as decision- making representative. The court may appoint from the nominees put forward, one or more than one panel member to be decision-making representative for the relevant person. Following appointment to the Panel, a panel member will be required to provide a resume of their skills, expertise and experience in a prescribed form. The DSS provide this resume to the court for consideration for the appointment of a decision-making representative. The DSS will contact the panel member before submitting this information to the court, to confirm the panel member’s availability for that specific case and ensure no conflicts of interest exist. Panel Member Nominee Attendance in Court ●     The court may request the panel members (two or more) nominated by the DSS from the Panel to attend court in-person or on-line for selection for appointment as decision-making representative. ●     The nominee selected for appointment by the court will be required to attend court in-person or on-line. ●     The nominee selected for appointment must attend court to sign a statement to confirm that they understand and will undertake the powers and duties conferred in the court order and will comply with the guiding principles. ●     Panel members are not entitled to reimbursement of any costs incurred as a result of attending court in the above scenarios. Statutory Functions of a Decision-Making Representative A panel member is appointed as a decision-making representative by way of a court order called a decision-making representation order (‘the court order’). The court order sets out the specific role and responsibilities of the panel member and any conditions that may apply. For each appointment, a decision-making representative must sign a statement in accordance with section 38(11) to confirm they understand and will undertake the powers and duties conferred in the court order and will comply with the guiding principles. The court order is the legal basis for all actions taken by a decision-making representative. The court order will outline the decision(s) the decision-making representative will have responsibility for and make provision for other matters as the court considers appropriate. The decision(s) that a decision-making representative will be required to undertake may involve property and affairs or personal welfare decisions or both. Personal Welfare Decisions Decisions about a person’s interests, health, and wellbeing. These types of decisions can include: (a)   Accommodation. (b)   Employment, education, or training. (c)    Social activities. (d)   Social services. (e)   Healthcare. (f)     Other matters about a person’s wellbeing. Property and Affairs Decisions Decisions about property, business, and money matters. These types of decision can include: (a)   Custody, control, and management of property. (b)   Sale, exchange, mortgaging, charging or gift of property. (c)    Acquisition of property. (d)   Carrying of a profession, trade, or business. (e)   Dissolution of a partnership. (f)     Carrying out of any contract. (g)   Discharge of debts, tax, and duty liabilities. (h)    Execution or exercise of the powers of a tenant for life. (i)      Providing for the needs of other people. (j)      Conduct of court or tribunal proceedings. (k)    Applying for housing, social welfare or other benefits. Reporting Requirement A panel member appointed as decision-making representative must prepare and submit annual reports to the DSS for each appointment on the performance of their functions which are within scope of the court order. Reporting requirements include details of all costs, expenses and remuneration claimed by or paid to the decision-making representative during the relevant period. Additionally, a panel member appointed as a decision- making representative to make decisions about the relevant person’s property and affairs must prepare and submit a report detailing the relevant person’s assets and liabilities, projected income and expenditure within three months of the date of their appointment by the court as decision-making representative. Eligibility RequirementsMandatory Applicants for the Panel must have the following: 1.      A qualification to at least level 7 of the National Qualifications Framework by Quality and Qualifications Ireland (or equivalent), which is to the satisfaction of the DSS appropriately aligned to the statutory role and functions of a decision-making representative and the range of property and affairs and/or personal welfare decisions that may be included in a decision-making representation order. AND 2.      Must be registered in Ireland with a professional regulatory body required by law. Proof of same shall be required prior to appointment to the Panel. AND 3.      Applicants must hold professional indemnity insurance (PII) to cover acting as a decision making representative, minimum PII requirement of €1 million. Proof of same shall be required prior to appointment to the Panel. AND 4.      Possess and demonstrate the competencies set out in Appendix 1.   Desirable ●     Relevant professional experience working with people who have decision-making capacity difficulties. ●     Knowledge of the Assisted Decision-Making (Capacity) Act 2015 or similar legislation. Training All applicants must complete training prior to being appointed to the Panel. The expected duration for the training is approximately 8 hours. Training will be delivered by a mix of online modules and self directed learning. Remuneration or the reimbursement of expenses will not be paid for undertaking this mandatory training.   Assignment to Area(s) The Panel will operate nationally, divided into eight (8) geographical areas aligned to the Circuit Court areas, detailed below. A panel member must be available to travel in the course of carrying out their function as a decision-making representative. Panel members will be assigned to an area based on the address provided for assignment purposes in their application form. The eight (8) DSS areas are as follows: DSS 1 Dublin DSS 2 Cork DSS 3 Kildare, Meath, Louth, Wicklow DSS 4 Laois, Longford, Offaly, Roscommon, Sligo, Westmeath DSS 5 Cavan, Donegal, Leitrim, Monaghan DSS 6 Carlow, Kilkenny, Tipperary, Waterford, Wexford DSS 7 Clare, Kerry, Limerick DSS 8 Galway, Mayo The DSS will endeavour to select panel members for nomination to the court for their assigned area. However, the DSS reserves the right to nominate a panel member to the court for a different area, where necessary and the panel member is available. The DSS will act reasonably in relation to the selection of panel members for nomination to the court, should such a requirement arise.   1.      Duration of Appointment If a panel member is appointed by the court as a decision-making representative, the time commitment required will depend on what is set out in the court order. While an appointment by the court may be for a limited purpose or duration, an appointment may also extend to a broad range of decisions and may potentially last for the lifetime of the relevant person/former ward. If a decision-making representative appointed from the panel needs to be discharged from their appointment due to unforeseen circumstances, they must make an application to court to be removed. Only the court can discharge a decision-making representative from their appointment. 2.      Remuneration Remuneration and certain expenses shall be paid to a decision-making representative in accordance with the 2015 Act, and as set out in the Assisted Decision-Making (Capacity) Act 2015 (Payment of Certain Expenses and Remuneration to Decision-Making Representatives) Regulation 2023 (the Regulations). The court order will set out the panel member’s entitlement to reasonable remuneration. A panel member may be paid out of the assets of the relevant person, or where the court so directs, by the DSS. The panel member must seek pre-approval for remuneration from the DSS at the beginning of each appointment. Annual limits apply based on the scope of the appointment. These are set out in detail in Appendix 2. In exceptional circumstances, subject to defined criteria, a decision-making representative may apply to be remunerated in excess of the applicable annual maximum. 3.      Specific Requirements Panel members will be required to comply with the following: a)     Panel members will be required to confirm compliance with various matters on an annual basis for continued membership of the Panel and these matters may change over the lifetime of the Panel. b)     Panel members will be required to sign a declaration as a condition of their selection for the Panel prior to appointment to the Panel and such declaration may be subject to change over the lifetime of the Panel. c)     Panel members will be required to report in writing all actual/potential conflicts of interest to include any involvement with any organisation, individual or other panel member (decision-making representative, special visitor, general visitor, and court friend) which could reasonably be perceived as constituting a potential conflict of interest in any case they are appointed. Panel members shall refrain from accepting nomination for appointment where there is a conflict-of-interest present. If a conflict-of-interest issue should arise at any point throughout the duration of an appointment as decision-making representative, the panel member must immediately notify in writing the court and the DSS. d)     Panel members will be required to adhere to a code of confidentiality and will be required to sign a declaration of confidentiality prior to appointment to the Panel or as required by the DSS. e)     Panel members will be required to adhere to data protection legislation and related policies and/or guidelines provided by the DSS. f)       Panel members must comply with tax clearance/ payment procedures as specified by the Revenue Commissioners. g)     Panel members must abide by the DSS Code of Conduct for decision-making representative panel members while fulfilling their appointed role. h)     Panel members must abide by the Assisted Decision-Making (Capacity) Act 2015, the Guiding Principles in the 2015 Act, the Code of Practice for Decision-Making Representatives (to be published under section 103 of the Act), the Code of Practice on Supporting Decision-Making and Assessing Capacity (to be published under section 103 of the Act) and any set of standards and related guidance issued by the DSS. i)       Panel members must abide by any applicable policies and procedures issued by the DSS and/or by the relevant Minister with regard remuneration/ reimbursement. j)       Panel members must maintain their professional registration in Ireland with the relevant professional regulatory body required by law. Panel members will be required to provide evidence of their registration prior to appointment to the Panel or as required by the DSS for continued membership of the Panel. k)      Panel members must hold professional indemnity insurance cover for continued membership of the Panel, and inform the DSS of any change in circumstances. Panel members will be required to provide evidence of professional indemnity insurance on an annual basis or as required by the DSS. l)       Please note panel members are required to set-up a designated mailbox for use only in your role as a decision-making representative. It will be used only as part of your role as a decision- making representative. 4.      Persons Ineligible to be on the Panel Having regard to the provisions of Section 39 and 40 of the 2015 Act and the requirements of the DSS in relation to eligibility and disqualification of a decision-making representative, a person shall not be deemed suitable to be on the Panel if that person: (a)   is an undischarged bankrupt or is currently in a debt settlement arrangement or personal insolvency arrangement or has been convicted of an offence involving fraud or dishonesty, (b)   is a person in respect of whom a declaration under section 819 of the Companies Act 2014 has been made or is deemed to be subject to such a declaration by virtue of Chapter 5 of Part 14 of that Act, (c)    is a person who is subject or is deemed to be subject to a disqualification order, within the meaning of Chapter 4 of Part 14 of the Companies Act 2014, by virtue of that Chapter or any other provisions of that Act, (d)   has been convicted of an offence under section 34, 80, 90 or 145 of the 2015 Act, (e)   is a member of the Mental Health Commission, (f)     does not hold the required qualifications, registrations, and professional indemnity, (g)   is not lawfully entitled to work in the State, (h)    does not receive Garda/security clearance, (i)      is convicted of an offence carrying a term of imprisonment. This means that instead of people being referred to a single service, they are now referred to a network of services and can access services within the network based on their need. All referrals to the network are received on a standardised referral form and are processed through a single point of entry through the Network Rehabilitation Coordinator role. Services would work together across organisational boundaries with people moving across the continuum of care seamlessly based on their needs. The MCRN model consists of a multi-tiered system of Neuro-Rehabilitation services and the Community Neuro-Rehabilitation Team (CNRT) provides a critical link in the care pathway by facilitating early discharge and continuity from both acute and post-acute rehabilitation facilities. The Community Neuro Rehabilitation Team is an inter-disciplinary therapy team providing a specialised service for adults with a range of progressive or acquired neurological conditions. This is a multi-professional team including a Consultant in Rehabilitation Medicine, Physiotherapy, Occupational Therapy, Speech and Language Therapy, Psychology, Social Work, Nursing, Dietetics and Administrative staff. This team will provide assessment and intensive therapy to people in both their own homes and in a clinic based setting for up to a period of 12 weeks. This service will provide people with earlier access to specialist rehabilitation close to home. The team provide a person centred service, improving people’s quality of life and working together to support people with neuro-rehabilitative needs. As this team is part of the Managed Clinical Rehabilitation Network, the newly established Community Neuro-rehabilitation Team will have a significant role to play with respect to evaluation of the service and contributing to further development of the team and the wider Managed Clinical Rehabilitation Network.
