The overall purpose of the RANP Older Persons Rehabilitation service is to provide safe, timely, evidenced based nurse-led care to Older People at an advanced nursing level. This involves undertaking and documenting complete episodes of patient care, which includes comprehensively assessing, diagnosing, planning, treating and discharging patients in accordance with collaboratively agreed local policies, procedures, protocols and guidelines and/or service level agreements/ memoranda of understanding. The RANP Older Persons Rehabilitation demonstrates advanced clinical and theoretical knowledge, critical thinking, clinical leadership and complex decision-making abilities. The RANP Older Persons Rehabilitation practices in accordance with the Code of Professional Conduct and Ethics for Registered Nurses and Registered Midwives (NMBI 2014), [MB6.1]the Scope of Nursing and Midwifery Practice Framework (NMBI 2015), Advanced Practice (Nursing) Standards and Requirements (NMBI 2017), and the Values for Nurses and Midwives in Ireland (Department of Health 2016). The RANP Older Persons Rehabilitation service provides clinical leadership and professional scholarship in the delivery of optimal nursing services and informs the development of evidence-based health policy at local, regional and national levels. The RANP Older Persons contributes to nursing research that shapes and advances nursing practice, education and health care policy at local, national and international levels. The RANP Older Persons Rehabilitation will address unmet needs with older persons with frailty by supporting the residential care services on campus. The main objectives of this RANP Older Persons Rehabilitation post at Peamount Healthcare are as follows: • To lead on the development of the Advanced Practice Older Persons nursing service at Peamount Healthcare in collaboration with nursing, medical and health and social care professional colleagues. • To manage an autonomous caseload of patients identified collaboratively within an agreed scope of practice (Domain 1). • To expertly assess the older adult, providing specialist geriatric opinion through holistic Cognitive Geriatric assessment (CGA) of the person including treatment and diagnosis at advanced level and refer to other health professionals as appropriate. • Accurately documenting assessment, diagnosis and integrated plan of care. • To combine skills and knowledge on a range of social, biological, and psychological variables in the provision of holistic care. The role will also include supporting families before, during and post diagnosis. The CDNT’s are charged with the delivery of an efficient and effective health and personal social service for children and young people with complex needs relating to a disability. DML Candidate Advanced Nurse Practitioner
Please note: The service serves a diverse adult population, with a significant focus on older people and vulnerable patients who wish to avoid unnecessary hospital admission. Specifically, the team manages patients with acute infections and complex clinical needs, this is not limited to the following examples of commonly referred conditions; osteomyelitis, pyelonephritis, septic arthritis, post-surgical wound infections, and complex skin or prosthetic joint infections. While the OPAT service & Community Intervention Services is already operational, a core focus of this cANP role is to lead its further expansion and the enhancement of clinical governance. A key future development is the evolution of clinical protocols to include the community-based removal of vascular access devices (such as PICC and mid-lines). This will create a more streamlined and comprehensive patient journey, allowing for the entire episode of care—from treatment initiation to final discharge—to be managed safely within the community setting. This team will also work closely with the development of the Neuro Rehabilitation Team, ICPOP and Chronic Disease Hubs. The unit consists of a specialist nurse-led team that operates as part of the wider primary care infrastructure. The service is clinically aligned with the National OPAT Programme and governed through the Public Health Nursing Service. The cANP works as a clinical leader within this multidisciplinary framework, collaborating with General Practitioners (GPs), Hospital Consultants, and the Integrated Care Programme for Older Persons (ICPOP) to ensure seamless service integration. The service covers the Longford/Westmeath geographic area, with its primary clinical base located at St Vincent’s Campus, Athlone, Co. Westmeath. The team operates across this region to facilitate early hospital discharge and prevent emergency department attendances for local residents.
