Community Healthcare Services are the broad range of services in a non-acute hospital setting, delivered as close as possible to people’s homes at the right time, in the right place, by the right team. There are 70 Primary Care Centres in the Region and specialist teams working in Mental Health, Health and Wellbeing, Social Care and Disability Services. Services are provided in various settings including the service user’s own home, day hospitals, community nursing units, hostels, specialist day care centres, training centres, respite care, and residential services. The region is served by 11 hospitals and the services delivered include inpatient scheduled care, unscheduled/emergency care, maternity services, outpatient and diagnostic services. St. Columcille’s Hospital is committed to providing a quality, patient focused service in a way that meets all patient needs in an equitable, efficient and safe manner. Services provided are: • MAU/LIU • Radiology • General Medicine including Gastroenterology, Gerontology, Respiratory Medicine, Endocrinology and Cardiology • General Surgery • Out-Patient Services • Weight Management Services • Pain Management Services The Hospital also provides Nutrition and Dietetics, Physiotherapy, Occupational Therapy, Speech & Language Therapy, Cardiac Diagnostic and Rehabilitation Services. The Sunflower Suite and the Inpatient Obesity Management Program: The 6 week inpatient obesity rehabilitation program aims to improve physical function and mobility and offer an opportunity for enhanced psychological treatments. It provides an opportunity to reset eating behaviours and to coordinate treatment for any medical complications of obesity that are present for example treatment of lower limb lymphedema or obstructive sleep apnoea. Each individual patient area consists of a bed, chair, table and storage facilities. There are two purpose-built toilet facilities within the unit. The nursing staff provide assistance with personal care should it be required. There are washing facilities available on site for laundry. The patients will follow a low calorie liquid diet consisting of semi-skimmed milk. The dietitian will discuss this with the patient in detail before admission. Taking part in the program is a great opportunity to establish a daily routine to build on mobility. There are various activities in which patients take part. Patients are encouraged to spend time outdoors in the space around the hospital grounds. The sunflower Suite is a four bedded unit, it is staffed by a nurse and a health care assistant. The department is supported by a Clinical Nurse Manager 2 (CNM 2) and an Assistant Director of Nursing (ADoN). The primary goal of the unit is to promote individual Nursing care to all patients. As demands on the service grow there is a requirement for the development of an evidence-based, high quality, adequately resourced service, dedicated to the care of patients and to the continuing education and development of staff. Reporting Relationship The post holder will: · Report to the Clinical Nurse Manager 2/ Assistant Director of nursing as relevant to the post/ as appropriate. · Be accountable to the Assistant Director of Nursing and Director of Nursing. Key Working Relationships The successful candidate will develop close working relationships with the patients, their families, Consultants and Doctors, Nurses, Physiotherapists, Occupational Therapists, Dieticians, Medical social workers, Speech and language Therapist, Patient flow manager, Admissions and Discharge coordinators. Purpose of the Post To be responsible for the management, care and treatment of service users, to ensure that the optimum standard of care is provided within the designated area(s) of responsibility. The primary role of the CNM 1 will be one of clinical and professional leadership and development in the nursing team, including the development of nursing staff by means of in-service training, orientation of new staff and arranging for clinical experience and supervision of student nurses where this is appropriate.
