This pathway for older persons supports better outcomes, reduced readmissions rates, reduced rates of long-term care use, reduced functional decline and improved quality of life. The senior post in Older Persons Services will support patient flow and the safe management of older persons in acute care, from the Emergency department and across to inpatient areas. The focus will be on providing early assessment and intervention for older persons and ensuring safe and timely discharge. The post will provide a 7/7 service ensuring that older persons receive the level of service required 7 days per week. The post holder will work with therapy colleagues as part of a roster.
