There is currently one 0.5 WTE (17.5 hours) permanent part-time vacancy available in the HSE South West, Community Neuro-Rehabilitation Team (CNRT). The inital base for this post is Mallow, Co. Cork. The CNRT is an inter-disciplinary therapy team providing a specialised service for adults with a range of progressive or acquired neurological conditions. This is a multi-professional team including a Consultant in Rehabilitation Medicine, Physiotherapy, Occupational Therapy, Speech and Language Therapy, Psychology, Social Work, Nursing and Administrative staff. This team will provide assessment and intensive therapy to people in both their own homes and in a clinic based setting for up to a period of 12 weeks. This service will provide people with earlier access to specialist rehabilitation close to home. The team provide a person centred service, improving people’s quality of life and working together to support people with neuro-rehabilitative needs. The Physiotherapist, Senior will be: Responsible for the provision and delivery of a high-quality, evidence-based Physiotherapy Service for persons with acquired and progressive neurological conditions in accordance with standards of professional practice. To work in conjunction with other team members in co-ordinating and developing the service to meet the needs of the population it serves in line with the objectives of the organisation. The post holder will work within a multidisciplinary team and provide Occupational Therapy assessment and intervention to service users within a designated clinical and geographical area. She/he will be responsible for supervision of a staff grade Occupational Therapist and development of the Adult Mental Health Occupational Therapy Service within their designated team. She/he will also work as a team member of the Occupational Therapy Service. There is a strong emphasis placed on supervision and continued professional development. The OT will be an active member of the Multidisciplinary Team (MDT). A variety of occupational therapy models of practice are used within the OT department including the Model of Human Occupation. There is a significant focus on the use of occupation based interventions, individually and in groups, to support the Service User’s recovery. The OT will carry a case load and will complete occupational therapy assessments & interventions looking at the impact of the person’s mental health difficulty on their occupational performance. As part of the MDT, the OT will be an active member in the weekly team meetings and contribute to the Individual Care Plan. OTs working in the service will be initially closely supervised and dependent on the individual therapists measured ability and experience to date. Occupational Therapists are considered as key members of the multidisciplinary team and are included in evidence based CPD opportunities and detailed complex case discussions. They contribute to and lead out on service development initiatives both within their teams and in the OT department. The OT will be supported and guided by an experienced OT manager who will provide supervision. Senior therapists will also provide supervision to staff grade therapists (depending on experience). Opportunity for peer supervision is also available. There are opportunities for departmental learning during the monthly OT meetings, CPD committee, Research Committees and a number of other service development initiaves that are supported by the OT manager.

The RANP (Heart Failure Acute Integrated Care 50/50) practices in accordance with the Code of Professional Conduct and Ethics for Registered Nurses and Registered Midwives (NMBI, 2014), the Scope of Nursing and Midwifery Practice Framework (NMBI, 2015), Advanced Practice (Nursing) Standards and Requirements (NMBI, 2017), and the Values for Nurses and Midwives in Ireland (Department of Health, 2016). The RANP (Heart Failure Acute Integrated Care ) service provides clinical leadership and professional scholarship in the delivery of optimal nursing services and informs the development of evidence-based health policy at local, regional and national levels. The RANP (Heart Failure Acute Integrated Care ) contributes to nursing research that shapes and advances nursing practice, education and health care policy at local, national and international levels. In line with Sláintecare (2017) and the Department of Health’s Capacity review (2018), a shift in healthcare service provision is now required to place the focus on integrated, person-centred care, based as close to home as possible.
