JOB PURPOSE: The purpose of this role is to undertake timely, clinically informed nursing assessments for all new referrals at the request of the chair of New Referrals Committee, Adult Day Waitlist and Residential Approvals committees as appropriate. Using nursing expertise, the postholder will evaluate clinical need, determine appropriate pathways, and support safe, effective admission, discharge and transfer decisions. Working collaboratively with multidisciplinary teams and key stakeholders. The role ensures that referrals are assessed holistically and makes recommendations for the most suitable service, promoting high‑quality, person‑centred care and optimising efficiency across the organisation.
