Heart Failure/Secondary Prevention Programmes are proven to improve quality of life and to reduce morbidity and mortality rates. The Clinical Nurse Specialist (CNSp) in Heart Failure will deliver a comprehensive Heart Failure Programme to patients over a phased period of time and in accordance with individual patient need. The CNSp plays a vital role in empowering patients with the skills and knowledge necessary for them to achieve optimal health and well being (Duffy et al, 2000).
