The Senior Medical Social Worker will be based in Sligo providing a service to persons in the Community Health Organisation (CHO) area 1 Sligo Leitrim and South Donegal . There is currently a permanent vacancy available in the Memory Assessment and Support Services (MASS). A panel may be formed as a result of this campaign for Social Worker, Senior Medical from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled. The strategic focus of care in CHO 1 is providing alternative pathways of care through the establishment of Integrated Ambulatory Hubs throughout the five counties. There are three ambulatory care team established in Sligo Leitrim, Cavan Monaghan and Donegal OPS consisting of Advanced Nurse Practitioners, Clinical Nurse Specialists Physiotherapists , Occupational therapists , Social work, Dietician, Speech and Language and Therapy assistants and will be expanding to include a wider network of multidisciplinary team members in all three hubs in line with Enhanced Community Care guidance document. The MASS will be operationalised in 2022 as part of the Integrated care programmes. One Operational Team lead will oversee the day to day operational aspects of all Older Persons Ambulatory teams in each of the three areas. This will include service such as Reablement / Falls prevention/ MASS / FIT and any further service developments that will be in operation in Older Person Services under the umbrella of Integrated Care. The Integrated Care of the Older Person’s governance groups (ICPOP) are already established with representatives from senior management across all divisions involved in the care of the older person. The main aim is to develop and implement integrated services and pathways for older people with complex health and social care needs, shifting the delivery of care from acute hospital towards community based planned and co-ordinated care. The role of a Fisheries Officer has two main areas of responsibility; REMUNERATION The salary scale for the position is at the level of Fisheries Officer as applies in Inland Fisheries Ireland, it is a 14-point scale including 2 long service increments (LSI’s): Point 1 €32,299.08; Point 2 €33,901.38; Point 3 €34,826.61; Point 4 €36,084.89; Point 5 €37,076.48; Point 6 €38,109.19; Point 7 €38,865.10; Point 8 €39,927.13; Point 9 €40,812.20; Point 10 €41,519.36; Point 11 €42,574.81; Point 12 €43,642.87; LSI 1 €44,45,188.81; LSI 2 €47,159.90 (IFI FO Grade Payscale as of 01.08.2025) An unsocial hour’s allowance (UHA) of up to €7,821 per annum (pro-rata), will be payable at either 50% or 100% relative to the number of unsocial hours worked. There is currently one whole-time specified purpose post available in Sligo, South Donegal/ Leitrim, West Cavan (SSD/LWC) Public Health Nursing Department. The post holder will be based at Markievicz House, Barrack St, Sligo. A panel may be formed as a result of this campaign for the Public Health Nursing Service, SSD/LWC from which current and future, permanent and specified purpose vacancies of full or part-time duration may be filled. The person appointed to this post will work within the Public Health Nursing Service, Sligo,South Donegal/Leitrim, West Cavan, CH CDLMS Structures and wider Integrated Services Directorate; working within Primary Care, as part of multi-disciplinary teams delivering a coordinated approach to client care. The Sligo, South Donegal/Leitrim, West Cavan Public Health Nursing Service provides a service across the life span to all client groups, and includes a School Health and School Immunisation Service, Community Intervention Team and provides professional governance for nurse specialists within the Chronic Disease Management Hub. The Assistant Director of Public Health Nursing will have responsibility for team of PHNs, RGNs and/or other specialist nursing staff within an assigned clinical network area. The PHN Service liaises closely with nursing colleagues in the acute, secondary, tertiary and end-of-life care services, and with General Practitioners and Practice Nurses within Primary Care Teams. Services are provided at the level of the individual, family and broader community services. In addition to delivering personalised care to individuals, account is taken of the collective health, well-being, social and nursing care needs of the whole population. The PHN Service is the only universally available community nursing service and is located in every community in Ireland, delivering care in diverse settings including the home, school, community and health centres. Activities include clinical nursing care, Immunisations, Health promotion and specialist Integrated nursing care in line with Slaintecare. Sligo, South Donegal/ Leitrim,West Cavan Public Health Nursing Service is a clinical learning environment for Post Graduate Student PHNs and Pre-registered Nurses.The post holder will be required to ensure that the learning environment and learning outcomes meet the standards required by An Bord Altranais and HEIs.
